0882447351 2015-04-05 Production Wells No Violations I was on site with DOGRM inspector Heidi Scott to perform a follow-up compliance inspection. We started on the north side of Antigua Rd SW and walked along the stream to the spring. The absorbent booms that were placed in the spring and at the outlet of the spring to the creek were saturated and needed replaced. There was slight oil sheen in the vegetation in the spring. We then walked the route of the ditch that had been dug to contain the oil, and there was oil sheen and some oil in the ditch. There was oil staining around the well head. Mike Melzer, contract pumper for Dover-Atwood, met us on site. We informed him that the contaminated soil along the ditch and around the well head needed to be removed, the saturated booms and diapers needed to be replaced with fresh ones. We then inspected the tank battery. It shared production equipment with PN# (946, 947, and 948), and consisted of two-210 BBL tanks, two mechanical separators, and one 50--Sorb had been placed over small sections. We could not locate a source of a leak at the time of inspection, and concluded that the source of the oil inside the tank battery was past over-flowing of tanks. There was a drainage pipe from the dike at the east side of the dike which contained no valve or plug. Oil sheen was present in the water draining from the dike, along the access road, and in the ditch. We spoke to Mike Melzer, the pumper, on-site about the tank battery. Dover-Atwood is to build-up the dike and remove the drain pipe, remove and properly dispose of all contaminated soil, locate and repair the source
1423600718 2015-01-23 Production Wells Violations Noted Northeast Regional Supervisor Tom Hill received a call from Geoff Finley of Banjo Inc., representing Dover Atwood. He indicated that he had discovered a flow line leak at the site, and that oil had been released into a ditch. Supervisor Hill and I went to the site. During our inspection, we found that the flow line had ruptured on a hill side, and oil had flowed down the hill side, then downhill into a drainage ditch. The oil appeared to travel approximately 230 feet downhill. We also found that near the well head there was another area of fresh crude oil, but not coming from the wellhead. It appeared that the flow line had leaked in two areas. The area of impact near the well head was about five feet by ten feet. The oil was thick due to the cold weather. It was also discovered that the oil in the drainage ditch had encountered some fractured bedrock, and flowed underground some 600 feet to a spring, where it exited into a small pond. The pond was entirely covered in a layer of oil, but was prohibited from traveling downstream by natural vegetation. Mr. Finley, following discussions with Supervisor Hill, mobilized equipment and secured supplies to begin remediating the spill. Mr. Finley was also advised to contact the Ohio Environmental Protection Agency (OEPA) and alert them that the spill had impacted a surface body of water. The OEPA was contacted. Remediation activities included digging of a sump at the drainage ditch, steam cleaning the oil to the sump, and removing the oil with a vacuum truck. Booms and absorbent pads were placed at the pond to absorb oil, and prohibit it from migrating downstream. Peat sorb was also used to help absorb the oil. Due to inadequate pipeline strength and well operation causing pollution and contamination, a Compliance Notice is being issued. The leak must be repaired, and the oil cleaned from the soil and pond and properly disposed of by January 30, 2015.
1432842787 2015-04-07 Production Wells Violations Noted Ashley Raff, Carroll County Inspector, and myself were on site to inspect the clean up of an oil spill that was discovered on 01/23/2015. The spill occurred when the flow line leaked in two spots. Oil had traveled down a ditch next to the lease road and had entered the ground midway down the lease road and followed a path into a spring located to the west on lower ground. Oil contaminated soil is evident at the well head where one leak occurred and on the slope east of the well where the second leak occurred. Oily peat sorb is in the ditch and an oil sheen is evident in the ditch water before the sump. The sump is still in place and oil is evident in the sump water. There is oil stained peat sorb downstream from the sump and an area in the ditch where the ditch water enters the ground and there is not oil in the ditch past this point. The spring has weathered oil and needs two water logged oil booms replaced. Contacted Atwood and requested they schedule further clean up ASAP. Mike with Atwood met us on site and replaced two booms in the spring and put diapers in the sump. Atwood is going to contract an outside clean-up crew to remove oil soaked peat sorb and oil stained soil. Inspection of the tank battery showed two rusty, 110 bbl. Tanks; 1 rusty 50 bbl. Tank, 2 mechanical separators and a meter run inside of a dike. The tank battery is located in a low, wet area. There are areas of standing water inside the dike, old oil mixed with peatsorb is present through out the dike. A connection is leaking oil at the production tank closest to the west side of the dike. A pipe is letting water through the north side of the dike and a low area of the dike is allowing water to escape from the south side of the dike. Atwood is going to remove everything in the dike and raise the grade so the tank battery is higher. They will replace the tanks with one production tank and one saltwater tank and rebuild the dike. District Supervisor Rick Anderson issued a compliance notice
1432911523 2015-04-20 Production Wells No Violations On site with Ashley Ruff, Carroll County Inspector, to monitor further remediation of the oil spill that occurred in January 2015 when the flow line leaked. J-Hawk is on site and is removing oil stained soil from the slope to the northeast of the well. District Supervisor-Rick Anderson said this area was pressure washed at the time of the original spill. J-Hawk also is removing oil stained peat sorb from the ditch that runs along the east side of the lease road. One dump truck of oil stained dirt was removed and lime was placed over the slope in the area of the soil removal when heavy rain started. The tank battery and well head clean up will need to be rescheduled for a later date. The well site will need to be evaluated in dry weather to determine if further work of the slope and ditch will be needed. It is recommended that the ditch and sump stay in place to catch any residual oil that may still come to surface.
1442104584 2016-08-09 Production Wells No Violations I was on site to conduct a compliance notice-follow-up with DOGRM district supervisor Rick Anderson. We walked the course of the drainage ditch up to the well. The leak has been repaired and all contaminated soil has been removed. The impacted ground has been remediated and vegetation was present. No leaks or contaminated soil were present around the well head. No oil or contamination was detected at the previously impacted pond. All booms and absorbent pads have been removed. The compliance notice can be closed.
