1242100274 2017-04-06 Production Wells No Violations The well is equipped with an electrically-driven pumpjack. A domestic connection with a drip bottle was present at the well. Unused tubing was located adjacent to the pumpjack. The tank battery is labelled as sharing production equipment with PN# 20756 & 20762 and consists of one 210 bbl production tank, one vertical separator, and one drip bottle. The tank battery is surrounded by a dike and the load line to the tanks production valves were equipped with locks and plugs. Identification was posted at the well and tank battery.
1332778685 2012-03-22 Production Wells Violations Noted I met with Inspector Brett Farnsworth (ODNR - Oil & Gas) and Tim Cottis (Carroll County EMA) to investigate the odor which was believed to be H2S. A Sperian gas Detector (Model PHD-6) was used to test for the H2S and no H2S was indicated by the meter. Gas is leaking from around the well head. Make the necessary repairs to the well head to prevent the gas from being emitted into the air.
