1431627833 2015-05-13 Production Wells No Violations I performed a status check on this well. The well is equipped with an electrically-driven pumpjack. The polish rod was lubricated and the production valves were open. The belt guard was missing, and no posted identification sign was visible at the well. The tank battery contained one 210-bbl tank and one vertical separator. The posted identification on the tank battery stated that it contained equipment for wells Gradhaus #2 and #3, but no permit numbers were posted. Flat plugs were present on the pick-up lines to the tank. There was a leak at the brine pick-up line that measured at a rate of one drip every four seconds. The dike was in good repair. There was a large fallen tree across the lease road that blocked access to the well and tank battery. A review of production records in the State database shows the last reported production for this well was 57 bbls oil, 27 mcf of gas, and 48 bbls of brine in 2014. I contacted Frank Periano, the pumper for this well, to discuss the missing permit numbers and fallen tree on the lease road. I was informed that Dover-Atwood is making arrangements to remove the fallen tree on the lease road this week. He was informed that proper identification is to be posted by the end of this month. I will follow up during the week of June 1st.
2000395729 2015-06-01 Production Wells No Violations I performed a follow-up inspection of this well. The fallen tree that blocked the lease road at the time of my last inspection had been removed. Proper and clear identification was posted at the well head and at the tank battery. The identification now stated that the tank battery contained the wells for PN#( 625 and 626). The dripping brine leak had been repaired, and no leaks were detected at the time of inspection.
