0515930110 2018-02-01 Production Wells No Violations I was on site to follow-up on complaint #5728 regarding contamination after a flowline leak. I did not locate any contamination at the source area. I also walked the course of the nearby ponds and did not locate any sheen or contamination in the water. The ground was covered with snow, and the area where the release occurred was in a topographical low-spot and filled with water and snow. I will further access the site during better weather and when the ground is dry before closing out the complaint.
0618869320 2018-12-11 Production Wells No Violations I was on site to follow-up to complaint #5728 which was called in after a flowline release on 10/19/2017, and another release occurred on 10/04/2018. The area of compromised flowline was reported as replaced. No visible contamination was present at the time of inspection. The well was equipped with a pumping unit and was operating at the time of inspection. No violations were observed. The complaint can be closed at this time.
0666629088 2017-10-19 Production Wells No Violations I inspected this well in response to complaint #5728 from the landowner who believes the area was not cleaned up properly after a flow-line break. I spoke to Enervest, who informed me that a flowline break occurred on 10/18/2017, and a non-reportable quantity of approximately under three barrels was released. I located the location of the release which was along the west side of the lease road and approximately 120 feet north of the well. The area had been excavated and patches of oil-stained vegetation were visible. I could not locate any mobile oil or further contamination. A pond is located approximately sixty feet east of the break, and I did not locate any areas of sheen in the water. The well was shut-in. I spoke to Bob Whaley, foreman for Enervest who informed me that the company will be out on 10/20/2017 to clean-up any remaining residue and remediate the soil at the point of release.
1721568236 2017-05-16 Production Wells No Violations The well is equipped with an electrically-driven pumpjack. The tank battery is labelled as sharing production equipment with PN# 20593& 20568 and consists of one 210 bbl production tank, one vertical separator, and one underground cement brine vault. I did not locate the meter run for the well. The load lines to the tanks production valve was secured with a flat plug. The tank battery was surrounded by a dike. The lease road was stable. Proper identification was posted at the well and tank battery.
1898258015 2017-10-26 Production Wells No Violations I was on location to follow-up to complaint #5728 regarding a flow-line break which occurred on 10/18/2017. The excavated soil at the location of release appeared to have been mixed with lime, and absorbent peat had been applied. The absorbent peat was dry. The area had experienced several days of rain since my last inspection on 10/19/2017. I did not locate any mobile oil or sheen in the pond adjacent to the lease road.
1943587341 2018-10-11 Production Wells No Violations I was on site to follow-up after a flowline failure occurred on 10/04/2018. The initial incident was followed up by DOGRM inspector Shawn Murphy. Incident remediate reports indicate the failure was remediated at that time. There is currently an open complaint on this well (#5728) for a flowline failure which occurred on 10/19/2017 at the same location. I called the foreman for Enervest to discuss why the line failed at the same point as it did in 2017. He stated that a dresser sleeve was installed on the line after the incident in 2017. After the most recent failure, Enervest removed three joints of compromised flowline and replaced with new pipe. He also informed me that during cleanup two loads of contaminated soil were removed and new topsoil was brought on site, which was then seeded and covered with straw. At the time of my inspection, no visible contamination was present.
