0260592623 2015-03-30 Production Wells Violations Noted

-210 bbl oil production tank, two-100 bbl. brine tanks, and a heater treater separator. Inspection of the tank battery revealed a label indicating the production equipment is shared by G & O Resources PN#(438, 436, 438). There was an oil stain measuring 76 inches by 14 inches surrounding one of the tanks, and there was free crude oil pooled at the base of the heater treater. The dike was breached and not sufficient for containment. Tank fluid leakage was evident from significant dead vegetation in the land adjacent to tank battery and in the pasture surrounding the dike. Oil has breached the dike and has pooled in an area measuring 20 feet by 12 feet. There was no bull plug on the valve of the production tank. The gas meter at the tank battery contained no chart. Identification with permit numbers was not present at the tank battery. RBDMS shows there has been no reported production for this well since 2010, which was 11 bbls. Due to well operation causing pollution, failure to have required bull plugs, the dike not able to prevent escape of brine, and proper identification not posted at the tank battery, a Compliance Notice is being issued. The oil and brine and impacted soil around the tank battery must be removed immediately, the leaks at the production tanks and heater treater must be repaired, the required bull plugs must be installed, and the dike upgraded by May 13, 2015.

0422360659 2018-11-12 Production Wells No Violations I was on site for a compliance-notice follow-up which was issued to plug or produce the well. I was unable to access the well due to heavy overgrowth of vegetation. I will contact the operator for the plugging status of the well and request the access road be cleared. No violations were observed at the tank battery at the time of inspection.
0831873386 2018-12-11 Production Wells No Violations I was on site for a compliance-notice follow-up to plug the well. I spoke to the production contractor for the operator on 11/30/2018 who informed me they will be moving equipment on site to service the well and expects the well to be put back into production in 30 days. At the time of inspection, no service equipment was on site and the production valve was in the closed position. Proper identification was posted at the well.
1177555438 2018-09-20 Production Wells No Violations The issues at the tank battery stated in the compliance notice issued on 03/30/2015 have been resolved. The tank battery now consist of one 210 bbl production tank. The load line to the tanks production valve contained a plug, and the dike was in good repair. The identification posted at the tank battery only displayed the well PN# 20438. At the time of inspection, I was unable to access the well. It was heavily overgrown with vegetation. The compliance notice issued on 03/30/2015 can be closed once confirmed proper identification is posted. A compliance notice was also issued 06/23/2015 for non-production. The last reported production was 11 bbls in 2010. I will meet with the operator to discuss a plugging schedule this well.
1357366938 2015-06-23 Production Wells No Violations I conducted a compliance follow-up inspection of the tank battery. All production equipment except the meter run, the valve system, and one, 100 barrel production tank had been removed. A new dike had been constructed and housed the valve system. The production tank located outside of the dike, and was not attached to any equipment at the time of inspection. All of the impacted soil had been removed.
1377549965 2013-08-23 Production Wells No Violations The well was surrounded by a puddle of water and the pump jack was very rusted. There was a rusted piece of equipment next to the pump jack. The gate to the lease road was locked and the road was overgrown with young trees and brush. There has been no production recorded for this well since 2010, and that last production was oil (11 barrels).
1446834158 2015-10-15 Production Wells Violations Noted I was on site with DOGRM Regional Supervisor Tom Hill to conduct a status check of this well. We did not inspect the tank battery during this time. The well was equipped with an electrically-driven pumpjack. The production valves were in the closed position. The polish rod did not appear to be lubricated. The joints and fittings to the pumpjack and the pivot points were corroded. The lease road was heavily overgrown and inaccessible by vehicle. Identification was posted at the well, but did not contain the required information. There is an open compliance notice issued on 4/22/2015 to post the required identification at the well. A review of records in the state database shows that the last reported production for this well was 11 bbls in 2011, and has not produced the regulatory-required minimum production since 2004. The well is not equipped for production at this time. A Compliance Notice is being issued to plug the well. The operator shall submit a plugging schedule to inspector Ashley Raff for approval at: Ohio Department of Natural Resources, Division of Oil and Gas, 3575 Forest Lake Drive Suite 150, Uniontown, Ohio 44685 within 30 days of 11/10/2015, per OAC 1501:9-11-05(B) and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1889938337 2013-08-22 Production Wells No Violations I was on site to today with Helen Halter, in regards to her complaint of idle wells on her property. I inspected this well with Helen. The well is shut in. Review of production records showed that last reported production was 11 barrels of crude oil in 2010. No production was reported for 2009 and 18 barrels of oil were reported in 2008. This well currently does not meet the requirements for a commercially productive well: ORC 1509.062. This well either needs put back into production or plugged. I will be having a meeting with the operator on 08/30/13 to discuss the issues with this well.
