0432843996 2015-08-19 Production Wells No Violations I met DOGRM hydrogeologist Kyle Tharpe on site to try to locate the source of contamination of the standing water in the tank battery. We tested the standing water inside of the tank battery and found the chloride level to be off the scale of the high range Quantab, therefore greater than or equal to 6364 ppm. We determined that there were no wells nearby that could have contaminated the drainage basin leading to the site. We were unable to locate the source of contamination at the time of inspection. There is a stream located approximately fifty feet to the west of the tank battery, but it was found that any contaminated water inside of the tank battery was contained and unlikely to travel into the stream. The inspection is ongoing while I consult with another inspector to try to determine the source of contamination so that we can institute a remediation plan.
0893682685 2015-08-03 Production Wells No Violations I conducted a status check of this well. The wellhead is equipped with an electrically-driven pumpjack. The tank battery consists of one 100 barrel production tank, one 50 barrel brine tank, and one drip bottle. The tank valves were sealed with a plug. The tank battery was surrounded by a dike, and was located above ground level. The tank battery is located adjacent to a slope, and standing water was present in the area between the slope and tank battery. I tested the standing water inside of the tank battery and found the chloride level to be off the scale of the high range Quantab, therefore greater than or equal to 6364 ppm. I was unable to locate any leaks or source of contamination. No oil sheen or scald areas were visible around the tanks. I will consult with another inspector to help determine the source of contamination at this site so that we can move forward with a plan for remediation.
0921007401 2015-08-27 Production Wells No Violations I was on site with DOGRM inspector John Fleming to oversee remediation of the tank battery. They had removed the standing water and dead vegetation behind the tank battery and were steam-cleaning the underground brine vault when I arrived on site. After the brine vault was steam-cleaned, the top half was removed to be disposed of. We inspected the bottom section of the brine vault after it was cleaned, and found no contamination of the tank or surrounding soil. I had to leave the site while they were in the process of crushing the bottom half of the brine vault and then filling in the area with sand and clay.
0961505992 2015-08-24 Production Wells No Violations I investigated this site with DOGRM inspector John Fleming to the source of contamination of the water inside of the tank battery. We tested the standing water and found the chloride level to be off the scale of the high range Quantab, therefore greater than or equal to 6364 ppm. We located an unused buried underground brine vault on the south side of the tank battery. We were able to test the water inside of the brine vault with a low range Quantab and found the chlorides to be in the range of 111 ppm. We therefore determined that the brine vault was unlikely to be the source of contamination. We contacted Don Himes, pumper for Enervest to meet us on site. It was found that the likely source of contamination was an over-flowed tank. We discussed a remediation plan to have them first remove all standing water and vegetation inside of the tank battery. They are to run steam water over the area to leach out any chlorides while subsequently pumping out this water. They are to excavate around and steam out the brine vault. The dike will then be re-built to prevent any water inside of the tank battery from escaping.
1345483737 2015-09-01 Production Wells No Violations I was on site to follow-up on the status of the tank battery after remediation had been completed during the week of 08/24/2015. The dike had been built up and was in good repair. The area on the north side of the dike where the brine vault had been removed has been filled in. No contaminated soil was detected in the area. Water had infiltrated the dike on the east side, which is adjacent to a steep slope. I tested the standing water with a high range Quantab, and found the chloride reading to be in the range of 3393 ppm. This is an improvement to the reading of greater or equal to 6364 ppm that was taken on 08/24/2015. I will continue closely monitor this site and take regular chloride readings for any changes.
1441033874 2015-08-27 Production Wells No Violations I was on location to observe the steaming and removal of the in-ground concrete vault. There was no noted contaminated soil around the vault area. There was dirt and sand used to fill the void from the removed vault area. We also flushed the ditch area. The dike around the tank pad was rebuilt. This location will be monitored on east side where the water collects and the chlorides were out of limits on the high Quantab reading. The in-ground vault Quantab readings were 178 ppm low range and 5-6 ph reading. ID signs are posted.
1443624008 2015-09-30 Production Wells No Violations I conducted a follow-up inspection on the tank battery. The dike was stable and in good condition. No leaks were detected at the production equipment. I tested the standing water in the tank battery for chlorides with a high-range Quantab, which gave a chloride reading of aproximately 3093 ppm. I will continue to moniter the standing water in the tank battery for chlorides.
1443962601 2015-09-24 Production Wells No Violations I conducted a follow-up inspection on this tank battery. I test the standing water for chlorides with a high-range Quantab which gave a chloride reading of approximately 4559 ppm. The dike was in good repair. No leaks were detected. The well was producing at the time of inspection. I will continue to moniter the condiiton of the tank battery and quality of the standing water.
1618922031 2015-08-24 Production Wells No Violations On site with Inspector Ashley Raff . She tested the standing water on the east side of the tanks and the chlorides were on the high range Quantabs. ( 8.8 ) ( 6562 ppm ). We also discovered an in-ground concrete vault that was used to drain salt brine into. The concrete vault is no longer used. Test results of the fluid in the vault with a low range quantab results were 4.0 on the low range ( 168 ppm). The ph. Reading was 5-6. We meet with Enervest Field Supervisor Don Himes and explained what needs to be down. Steam and remove in-ground vault clean leafs out of ditch and flush ditch with hot water. The tank battery is equipped with one 210 barrel tank and one 50 barrel poly tank. There is no production unit. The well is pumping fluid only.
1657148926 2007-05-01 Production Wells No Violations Inspect wellsite for the Huff Run Watershed Project. Inspection of the wellsite and the tank battery revealed no violations at this time. The tank battery consists of one 100 bbl oil storage tank and one buried cement brine storage tank. The SPCC dike is not adequate at this time ; not high enough .
