0454120352 2015-06-15 Plug / Plug Back No Violations I was on the James Brothers Coal Co. # E-1 to witness the setting of the top surface plug. Chipco Service was the plugging contractor and cement contractor on the hole. Chipco removed the 7 overshot with 71 of 4.5 casing attached. Production casing recovery was not attempted due severe corrosion of the 4.5 and possible loss of hole. A string of 1.5 was lowered down the 8.625 casing and outside of the 4.5 casing to a depth of 236. The 1.5 tubing was determined to be outside the 4.5 with the indication of the collars tagging the top of the fish every 33 ( joint length ) and then catching the 4.5 collar 8 deeper. Chipco established circulation with 28 barrels of water ( Temp. @ 72 F, pH @ 8 ) and circulated 146 sacks of Class A back to surface. The cement top was topped off twice after falling down hole. After 30 minutes cement top was found to be 2 down from grade. No violations identified during this inspection.
0455963423 2015-06-16 Plug / Plug Back No Violations I was on the James Brothers Coal Co. # E -1 to examine the cement top after setting the surface plug the previous day. The cement top was found to be at 49 down in the 8.625 casing and solid. After cutting the casing off 30 below grade the cement top will be 19down from the required I D cap. Chipco is to cut and install the I D cap. No violations during this inspection.

0711379834 2015-06-09 Plug / Plug Back No Violations I was on the James Brothers Coal # E - 1 location to observe to plugging and abandonment of the well. Chipco Services was the plugging contractor and was removing the 2.375 milling string after having obtained a depth of 5000. Appalachian Well Surveys (AWS) perforated the 4.5 casing due to it being parted and its integrity was in question. The 4.5 casing was perforated from 3200 to 3350 with 75 shots ( 0.49 entry hole and 12.6 penetration ) at 2 shots per foot with 180 degree phasing. AWS also perforated the interval from 966 to 1053 with 46 shots ( 2/ ft & 180 degree phasing ). Chipco started to run the 2.375 tubing in the 4.5 casing in order to spot the Clinton sand plug. The well was shut in and secured overnight. No violation identified during this inspection

1433782100 2015-06-05 Plug / Plug Back No Violations I was on this wellsite to observe the plugging and abandonment of the James Brothers Coal Co. # E-1 well. Chipco Services is the plugging contractor. Chipco lowered 2.375 tubing in the 4.5 casing with a tapered mill on the end. An obstruction was encountered at 3170 and was milled through the tight spot. After milling through the obstruction the string gained an additional 15 m lbs. on the weight indicator. Chipco continued to mill inside the 4.5 until reaching approximately 3700 and then shut in the well for the weekend. No violations.
