0522576049 2018-11-06 Final Restoration No Violations I was on site for final restoration. The well was plugged through the DOGRM I/O program. Greater than 70% grass cover was present on the access roads and well site. All flowlines were removed by the contractor. There was no surface expression of the well and all debris and plugging equipment has been removed from site. Final restoration can be passed.
0633048063 2017-01-26 Orphan No Violations Todays inspection was conducted with Terry Whitehair and Fred Romeo from the Divisions Safety section. Safety is conducting their inspection as part of the technical review for the Orphan Programs pre-bid process. Mr. Devitt, the landowner, has applied to have the Orphan Well Program plug four Berea wells located on his property. See the Devitt Orphan Well file and prior inspection reports.

0834050646 2017-05-19 Orphan No Violations Todays inspection was conducted to try to determine if the 8 inch or 10 inch casings are intact as reported on the well card. 6.25 inch diameter casing is the only visible casing above ground level. Steel casing was found below ground level. An open annulus full of water was evident on the north side of the well, outside of the exposed steel casing. The multiflora rose will need cut down on the south side of the well to expose the entire casing to determine the casing diameter and if both the 8 inch and 10 inch casings are still in place. This well is enrolled in the Orphan Well Plugging Program.

1460404896 2015-11-04 Orphan No Violations Complaint #5168. The landowner, Mr. Devitt had originally contacted the Division during the 1990s, in 2005 and on 10/28/2015 requesting that four inactive Berea wells on his property be placed into the Orphan Well program for plugging. Mr. Devitt does not know who was the last well owner and said the wells have not produced since he became the landowner in the early 1990s. Todays inspection showed a Berea well equipped with a rusted grasshopper pump jack that operated with a Shackle line operation. The well has evident 6 casing with a casing head with 2.45 tubing and 1 rods. A gas packer may be in the well, but the gas line is not present. The ports are open on the casing head. A gas odor is detectable near the well head, the gas scope detected 3% LEL, 0 H2S near the tubing, casing head, 0% LEL and the casing head ports. The Divisions records list 21 of 10 casing; 169 of 8 casing; and 898 of 6 casing were installed in the well. The flow line and shackle line are intact and on the ground running towards the single tank by Permit 2-0030. This well, Permit 2-0030 and Permit 2-0062 shared the same Shackle works equipment and tanks. The multiflora rose inhibit determining if the entire flowline is intact. The 4th well, permit 2-0043, has its own tank. This well and the other 3 are not in operation. See the 11-04-15 inspection report for permit # 2-0030 for tanks and equipment affiliated with this well.
