1390315270 2013-10-21 Orphan No Violations I completed this inspection on 10/21/2013 to gather information for an Orphan Well Emergency Plugging of an historical oil well. This well is suspected to have impacted one of the property owners agricultural water wells, based on the results of a water well investigation conducted by the DOGRM hydro geologist Kyle L. Tharp on September 23, 2013. This water well has been temporarily decommissioned.
   I met the property owner, Joe Vahalick, and he showed me the location of the oil well and the potentially impacted water well, both located in an active pasture. 

Findings are as follows:

1431242674 2015-05-04 Plug / Plug Back No Violations Appalachian Well Survey ran bond log and CCL. Bond log revealed a TD of 1234 feet and no free pipe. Decision was made with Mgr. Chini, to not try to retrieve any of the casing. Decision was made to perforate the casing with four shots at each depth of 160 feet, 130 feet, and 100 feet to insure we get cement behind the casing at the approximate depth of the water well. We ran 39 joints of 2 3/8 EUE tubing to a depth of +/- 1220 feet without incident. Scheduled to set bottom plug at 8:00AM.
1647236669 2015-04-28 Plug / Plug Back No Violations Moore Well Service moved rig onto location, rigged up and proceeded to pull rods. Pulled +/-
1661407049 2015-05-05 Plug / Plug Back No Violations Petro Set Cementing ran 65 sacks Class A cement, with LCM for a 400 foot plug. Plug was down at 9:30AM. While we waited on cement to set, Canton Repository reporter Shane Hoover and photographer were on location along with Steve Irwin to interview concerning orphan wells.

At 1:30PM, the cement was still not set adequately. Decision was made to shut down for the day and resume in morning.
