0705177334 2018-08-23 Final Restoration No Violations I conducted a final restoration inspection of this well which was plugged to surface through the DOGRM Idle/Orphan Program on 06/12/2018. The well was cut 36 below grade on 06/13/2018 and there was no evidence of leaking. The area has been graded and vegetation is established. Final restoration can be passed.

1450279120 2015-11-04 Orphan No Violations Mr. Devitt, the landowner, contacted the Division regarding a water quantity problem. Mr. Devitt operates a large chicken farm and has problems obtaining water from the wells near the barns. The Divisions well map shows two Berea wells in the vicinity of the chicken barns. Division records show this well as abandoned on 9/30/1939 and completed on 04/25/1940. Mr. Devitt found an open hole during the excavation of the most northerly chicken barn. A large 14 diameter hole with no evident casing is behind the most northerly barn at the west side. There is no fluid in the hole and no evidence of gas. Mr. Devitt has a large rock over the hole. Mr. Devitt does not know how deep the hole was initially but the hole was partially filled in during the excavation process. He stated in a 11/17/2015 letter that the hole was quite deep and that it seemed to take a long time for a rock to land when a rock was dropped in the hole. The rock appeared to land in water according to Mr. Devitt. Division records state the well was completed to 1406 into the Berea and had 150 of 10 casing, 220 of 8 casing and 1010 of 6 casing. Kyle Tharp, Division Hydrologist, stated the hole is now only open to 810. There is not water in the hole at this time. There are four unplugged Berea wells that are evident and not in production that remain south of Chase Road on Devitts property.
