1336837265 2015-11-04 Orphan No Violations Complaint #5168. Mr. Devitt, the landowner, reported 4 non-producing Berea wells on his property. He reported the wells to the Division after taking ownership of the wells in the early 1990s, in 2005 and again in October of 2015. He does not know who was the last well owner. Inspection showed a Berea well equipped with an older pump jack. The engine is missing. Rods and tubing appear to be intact. The tubing has come apart below the connection for the flowline. A plate is inside the exposed 12 collar around a 2.4 OD tubing and a 1.4 OD gas packer. The Geo Survey Well card shows the well was drilled in 1939 is 1400 deep and has 50 of 10 casing; 195 of 8 casing and 960 of 6 casing. Its not possible to determine if the 8 and 6 casing are intact due to the plate. The flowline and gas line are not evident. An old drilling bit is on the back of the pump jack for weight. A piece of tubing/flowline is laying on a 10 piece of casing in the ground NE of the well, old oil is present inside the casing. A 100 barrel-?, rusty, vertical seamed steel tank is slightly inside the woods line 126 SE of the well. A slight gas odor is near the well. The gas scope detected 4% LEL near the well head. The well is not in production. This is one of four unplugged Berea wells that are existent on the Devitt property. The wells were never produced by the current landowner. Mr. Devitt would like the Orphan Well Program to plug the 4 existing Berea wells on his property. Permit #s 2-0030, 2-0080 are off of Patriot Road. Permit #2-0062 is through the field on the south side of Chase Road.

Safety is conducting their inspection as part of the technical review for the Orphan Programs pre-bid process. Mr. Devitt, the landowner, has applied to have the Orphan Well Program plug four Berea wells located on his property. See the Devitt Orphan Well file and prior inspection reports.
