0077578170 2018-08-10 Plug / Plug Back No Violations This Berea orphan well was plugged through the DOGRM Idle/Orphan program. Steve Harley was the contractor, and he contracted the service rig through Harley Well Service (HWS). Petroset was on site to set plug #1 from 1288---down until flow stopped, and a total of 70 bbls of fresh water were circulated to ensure static conditions were met prior to cementing. The plug was set with 115 sacks of Class A cement. I weighed the cement at 15.4 ppg. Mix water was 72 degrees F, and chlorides were <256 ppm. HWS removed the 2--down overnight.

0491094923 2018-11-06 Final Restoration No Violations I was on site for final restoration. The well was plugged through the DOGRM I/O program. Greater than 70% grass cover was present on the access roads and well site. All flowlines were removed by the contractor. There was no surface expression of the well and all debris and plugging equipment has been removed from site. Final restoration can be passed.

0989405732 2015-11-04 Orphan No Violations Complaint #5168. The landowner, Mr. Devitt, contacted the Division in the 1990s, 2005 and on 10/28/2015 requesting the Division place 4 inactive Berea wells in to the Orphan well for plugging. The wells have not produced since Mr. Devitt took ownership of the property in the early 1990s. He does not know the last well owner. Inspection of the well showed a Berea well equipped with an old lift pump jack that was connected to a Shackle line operation at one time. The pull is still connected to the pump jack. Tubing, rods and a gas packer are evident inside the 6 casing. A bent, rod is sticking up above the pump jack head. The flow line and gas line from the gas packer are still connected to the well head. The well is not in production. Division records show the well was completed on 9-14-38 to 1342 deep and produced from the Berea and the Big Injun. The well record shows 30 of 10 casing; 180 of 8 casing and 910 of 6 casing were in the well initially. A gas odor is evident near the well head. The gas scope detected 3%LEL/0ppm H2S at the well head. A single riveted, rusty tank is on the east side of Patriot Road, slightly off the road and west of this well. Coordinates for this tank are above. A second seamed tank is east of Patriot Road, in the woods at 40.54028/-8095838. A farm lane leads to the second tank. The seamed tank has two slow oil leaks. One leak is at the line connection and one leak is in the line. The bottom seam of the tank is moist near the valve. Approximately 5 x 3 of old oil is under the leaks. The manhole on top of the tank does not have a cover. Thick oil with either water or a lighter oil on top lay inside the tank. The amount of oil is unknown but it appears to be less than a foot. A barrel is on top of the seamed tank and pieces of line are laying on the ground near the tank. The seamed tank was connected to the riveted tank by the road. 1239375383 2017-01-26 Orphan No Violations Todays inspection was conducted with Terry Whitehair and Fred Romeo from the Divisions Safety section. Safety is conducting their inspection as part of the technical review for the Orphan Programs pre-bid process. Mr. Devitt, the landowner, has applied to have the Orphan Well Program plug four Berea wells located on his property. See the Devitt Orphan Well file and prior inspection reports.
