0934848707 2019-06-04 Casing String Witnessed No Violations On the day of my inspection I witnessed Cellar Tech had drilled a 40-inch diameter hole to the depth of 100 feet. Cellar Tech ran 101 feet of 30-inch 118.65 pound per foot Conductor Pipe with 1 foot of stick up above ground level. Lightning Energy Services was on location, pumped 41 barrels 195 sacks of class A cement with 2% Calcium Chloride and .25 pound per sack of poly flake with a weight of 15.6 pounds per gallon and a yield of 1.18 cubic foot per sack, with cement weight verified by mud scales. At 41 barrels into the cement job the pump pressure increased to 240 pounds per square inch and caused the casing to lift out of the well bore. When the casing lifted it broke one of the chains that was attached to the cellar tech cellar as well as a welded ear attached to the casing. The 30-inch diameter conductor was lifted to approximately 20- 30 feet into the air and returned to its original setting place. There were no reported injuries resulting from the incident. There were no returns of any kind to surface. I spoke with EQT construction supervisor Ben Wiemer and informed him that he will need to submit a written proposal for remediation. Ben informed me that they intend to run one-inch diameter tubing down the annulus of the 30-inch diameter casing and cement to surface. He will submit an incident report and remediation plan before moving forward.
