1525964207 2018-05-03 Casing String Witnessed No Violations The purpose of my inspection is to witness the 13.375 inch diameter surface casing being cemented within a 17.5 inch diameter borehole. A pre-job safety meeting was held with all personnel on location. Halliburton pumped 50 barrels of gel, 10 barrels of fresh water, and 322 barrels of cement. The cement was Class H cement weighing 16.4 pounds per gallon. I witnessed Halliburton pull a cement sample and weigh it using pressurized scales to confirm the weight of 16.4 pounds per gallon. The plug was dropped and 223 barrels of fresh water was used for displacement. An estimated 43 barrels of cement returns returned to surface. The plug was bumped and pressure was increased 500 pounds per square inch and held for 5 minutes. Floats held and 2 barrels were bled back to the truck.
1768718981 2019-02-07 Preliminary Restoration No Violations Preliminary Restoration status check inspection found The well site meets ORC 1509.072 with vegetation established and erosion control in place. The well site boundary sedimentation control has been removed. The well pad and lease road surface is stone construction and in good repair. The well pad perimeter is protected by an earthen berm, there is also an 8 chain link fence present around the well pad. There are 5 wells present on the pad. Identification is posted on each well and production tank along with the entry point of the lease road and well pad. The well is equipped with a lower A section and upper B section with 2 diameter pipes present attached to the service ports of both sections for the purpose of annular pressure monitoring. The riser pipes are equipped with valves and both electronic transducers and manual gauges. The pressure on the well head A section is 257 pounds pressure and the pressure on the B section is 1,501 pounds pressure. The 5.5 diameter production casing well head flow cross pressure is 1,713 pounds pressure and the pressure on the production head flow cross section is 1,266 pounds pressure. The well is selling off the 5.5 diameter production casing through (2) primary master valves with a production tree installed on the master valves, a single production flow lines is present, the pressure on the flow line downstream from the pressure reducing choke valve is 1,178 pounds pressure. There are 5 gas processing units on location and (2) production fluid storage tanks situated inside a polymer lined steel containment dike area. There is a Mark West mid stream pipeline service point at the left rear corner of the well pad. Emergency shut down devices are present at each critical area of the well pad including the entry point of the location.
