0527327558 2018-09-18 Casing String Witnessed No Violations On the day of my inspection I witnessed the top out of the 24-inch diameter well bore that Appalachian Drilling had drilled to a depth of 556 feet. Appalachian Drilling ran 539 feet of 18.625-inch diameter J 55 casing at 87.5 pound per foot, with 2 feet of casing above Kelly Bushing, which is 13 feet above ground level. Two Cement baskets were set at 437 feet. C&J Energy Services had pumped a 187 barrels 869 sacks of Class A cement at 15.7 pounds per gallon with a yield of 1.2 cubic feet per sack mixed with 5.25 gallons of water per sack. After speaking with XTO company man Steve Guile he informed me that at approximately 130 barrels into displacement they lost circulation. Appalachian Drilling ran in the annulus of the 18.625-inch diameter casing with 1-inch diameter tubing and tagged cement at approximately 220 feet. C&J Energy Services rigged up their cement lines to the 1 inch diameter tubing and pumped 78 barrels of Class A cement with 5 barrels of good cement to surface.

0926241817 2018-08-09 Casing String Witnessed No Violations On the day of my inspection I witnessed Lightning Energy had drilled a 36-inch diameter hole to the depth of 100 feet. Lightning Energy ran 104 feet of 30-inch diameter 117 pound per foot Conductor Pipe, with 4 feet of stick up. C&J Energy Services was on location rigged up their cement lines pumped 5 barrels of fresh water and performed a 500 pound per square inch pressure test, pumped 25 barrels of gel, washed up with 5 barrels of fresh water and cemented pipe with 66 barrels, 309 sacks of class A cement with 3% Calcium Chloride and .25 pound per sack of poly flake with a weight of 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement weight was verified by mud scales, Cement was displaced to 90 feet with 78 barrels of fresh water, with 20 barrels of good cement to surface. Mix water was tested revealing 51 parts per million chlorides, with a PH of 7 and 72 degrees.
1336419536 2018-11-21 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson Rig 579 had drilled the 12.25-inch diameter hole to 7915 feet and was currently tripping out of the hole at 3200 feet. After speaking with company man Gary Moore, he informed me that they were having trouble downhole steering and were coming out of the hole to change out the bottom hole assembly. I performed a walk around inspection on the well pad and noticed that a water truck was currently cleaning up approximately 2 gallons of oil base mud on the southeast corner of the well pad, the water truck driver informed me that the gasket on the suction line had leaked. I informed XTO company man Gary Moore that they needed to clean up and remove the affected area and dispose of the waste. No other violations were found at the time of my inspection.
1537873161 2019-01-15 Completion Testing No Violations Status check inspection found the well drilling process complete and the well being prepped for the stimulation process. Surefire Wireline is on location and has ran a caliper log on the vertical section of the 5.5 diameter production casing and is preparing to run a bond log. C&J Pressure pumping is slated to mobilize on location and begin stimulation in the next 14 days.
