0435012137 2018-08-08 Production Wells No Violations On site inspection accompanied by ODNR Inspector Ryan Armstrong found the well drilling and completion process now complete. The well is in the initial flow back production stages flowing trough a sand trap separator and choke manifold system, there is a 500 barrel horizontal tank present the sand trap separator is being discharged into. Annular pressure monitoring is not in place on the well head A or B sections.
0459178260 2018-11-05 Preliminary Restoration No Violations Preliminary Restoration status check inspection finds the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad and lease road surface is stone construction and in good repair. There are 5 wells total present on the pad with the # 2 & #4 being previously drilled and the #1-#3 &#5 being drilled in the 2nd round. The said well has no annular pressure monitoring or identification posted and is being flowed through a vertical sand trap separator with the fluid being ran into a temporary storage area consisting of (2) 500 barrel horizontal tanks. There is 3 gas processing units present on the pad. The permanent production fluid storage area consists of (4) steel tanks capable of holding 400 barrels fluid volume each and one horizontal 100 barrel tank, all situated inside a polymer lined steel containment dike area.
0492639765 2018-08-27 Production Wells No Violations On the day of my inspection I witnessed that the well was currently in flowback operations with 2 select flowback sand traps, and a choke manifold, there was also two 500-barrel fluid storage tank hooked up to flowback equipment. The well appeared to have gauges monitoring annular pressure but were underneath water and I was unable to read them. Production equipment included an individual Gas Processing unit and 4- 400-barrel production fluid tanks, and a 100-barrel production fluid tank shared with the other producing wells on the pad. There was a small leak on the 9.625-inch diameter casing, I spoke with EQT production superintendent John Allander and he informed me that they would send someone out to remediate the problem. Tim Hendershot arrived on location and evaluated the problem and dispatched a water truck to the pad to pump out cellars and remediate the leak. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current.

The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag were not present, I tried to contact EQT production superintendent Jared Lowery to inform him that they needed to be monitoring annular pressures and wells needed to be identified.
0849545915 2018-01-18 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 582 getting ready to start drilling their 17.5 inch diameter hole. Containment is functioning this date.
1031809213 2017-11-20 Casing String Witnessed No Violations On the day of my inspection I witnessed express had augured a 36 inch diameter hole to 100 feet. 100 feet of 30 inch diameter conductor casing was run. Express was onsite with 275 sacks of class A cement. Additives include 2%cacl and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 30 percent excess ran. Cement was displaced with 75 barrels of fresh water. 15 barrels of cement returned to surface. This is a replacement permit for iron warrior 3 (34013211740000) which was plugged and abandoned on 11/16/17.

1563791409 2017-12-29 Casing String Witnessed No Violations On the day of my inspection I witnessed the mine string cement job. Patterson 582 drilled a 24 inch diameter open hole to a depth of 499 feet KB. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet GL. Costys ran 462 feet of 20 inch diameter casing, j-55, 94 lbs/foot. Casing was centralizer with 4 centralizers. Shoe track is 46.5 feet long. Cement baskets were set at 387 feet. Coal encountered from 407-413 feet. BJs was onsite with 871sacks of class A cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 155 barrels of fresh water. 35 barrels of gel pumped ahead of cement. 30 barrels of cement returned to surface. Floats held with 4 barrels back to truck.

1953352999 2018-01-22 Casing String Witnessed No Violations On the day of my inspection I witnessed Patterson 582 run 480 feet of 1 inch diameter tubing down the backside of the 13.375 inch diameter casing to top out failed cement job. On Saturday 1/20, BJs pumped 1239 sacks class H lead cement and 731 sacks of class H tail cement and displaced with 55 barrels of fresh water when pressure spiked and job locked up. They ran a cement bond log and got permission from ODNR to top out with the 800 feet of 1 inch diameter tubing they ran on 1/20. Today, they established circulation through the 800 of tubin however the pressure was 900 psi pumping freshwater at .5 barrel per minute. The ran another string of 1 inch tubing to 480 feet and pumped 105 barrels of class A neat cement. 20 barrels of good cement returned to surface weighing 15 ppg.
1960908105 2018-06-21 Completion Testing No Violations Status check inspection on the well drilling process found the well is in production and producing off the production casing through the completion stack. The stack is owned by Silver Creek and consists of 2 primary manual master valves and a hydraulic master valve. No production tubing will initially be set in the well bore. The well is selling into a gas processing unit. No annular pressure monitoring is in place on the well. Also on site is a production facility consisting of 4 tanks of steel construction design situated inside a polymer lined steel containment dike area. There are 2 wells on the pad that were previously drilled and completed identified as the Iron Warrior # 2 and # 4 wells. 3 new wells were drilled and are in the initial flow back production stages being produced through choke manifolds and sand trap separators, these wells are identified as the Iron Warrior # 1-3 & 5 wells.
