--inch diameter 118.65 pound per foot Conductor Pipe. Petroset was on location rigged up their cement lines and performed a 400 pound per square inch pressure test, gained circulation with 5 barrels of fresh water and cemented pipe with 315 sacks of class A cement with 2% Calcium Chloride and .25 pound per sack of poly flake with a weight of 15.6 pounds per gallon and a yield of 1.18 cubic foot per sack. Cement weight was verified by mud scales, Cement was displaced to 90 feet with 76.5 barrels of fresh water, with 10 barrels of good cement to surface. Mix water was tested revealing 45 parts per million chlorides, with a PH of 6.5 and 72 degrees.

0946792987 2018-07-24 Drill / Deepen / Reopen No Violations On site inspection on the well drilling process found Falco Drilling rig 20 on location and rigging up on the said well and preparing to spud the 24 diameter surface mine string well bore. A know mine void is present at 443. The company men on site have been advised of the permit condition requirements to run a DV tool and swell packer. I also spoke with my Supervisor Brent Bear and the Division Permitting Supervisor Steve Opritza regarding the setting of the down hole equipment and the permit condition requirement.

1200869454 2018-08-16 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that the well was equipped with the A section of the well head. The cellars had been cemented up to the A section of the well head, and identification tags had been placed on top of the well head. Ascent Resources Drilling Coordinator Doug Bennett informed me that Patterson UTI 584 would be moving on location after it finishes in Jefferson County. There were no violations at the time of my inspection.
1532585306 2018-07-26 Casing String Witnessed No Violations On the day of my inspection I witnessed that Falcon Rig 26 had drilled the 24-inch diameter hole to a depth of 505 feet. Franks Casing crew ran 497 feet of 18.625-inch diameter J 55 casing at 87.5 pound per foot, with 5 feet of casing above Kelly Bushing, which is 13 feet above ground level. C&J Energy Services was on site and rigged up their cement head, loaded lines with 5 barrels of fresh water and pressure tested to 2000 pounds per square inch. C&J Energy Services established circulation and pumped a 20-barrel gel spacer with cell flake washed up and then pumped 188.6 barrels 905 sacks of Class A Cement, at 15.6 pounds per gallon with a yield of 1.17 cubic feet per sack mixed with 5.01 gallons of water per sack. Cement was displaced with 145 barrels of fresh water. Plug was bumped at 125 pounds per square inch and pressured up to 725 pounds per square inch and held for 5 minutes, floats held with 1 barrel of flow back. There were no cement returns to surface. Fluid level was visible at approximately 50 feet. Falcon Rig 26 then ran 1-inch diameter tubing down the annulus of the 18.625-inch diameter casing and tagged cement at 162 feet. C&J Energy Services then pumped 10 barrels of cement through the 1-inch diameter tubing and cement came up to approximately 118 feet. Waited on cement for 1 hour and tagged cement again with no loss in height. C&J Energy Services then pumped 72 barrels of cement with 15 barrels of good cement returns to surface. I informed Ascent company man Donnie Duran that casing must remain static until cement reaches 500 pounds per square inch compressive strength.
1547606779 2018-07-24 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that Falcon Rig 26 was currently on location and rigging up on the well bore. I spoke with company man Tony Shepard and he informed me that they plan to spud the 24-inch diameter well bore on 7/25/2018 at approximately 18:00 and drill to approximately 490 feet and run 18.625 inch diameter to total depth, and cement to surface.
1772767516 2019-02-26 UrbanPreliminary Restoration No Violations Preliminary Restoration found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad perimeter is protected by an earthen berm with a 8 chain link fence present on the berm. The well pad and lease road surface is stone construction and in good repair. Signage is posted along the lease road. 2 wells have been drilled and not completed. No production equipment has been placed on the pad to date.
