1258088351 2018-07-31 Production Wells No Violations Status check inspection fount the well drilling and completion process now complete and the well head equipped with a lower A section and upper B section, riser pipes with gauges installed are present on both sections to monitor annular pressure. The pressure on the 13.375 (A) section which is the pressure between the 13.375 and 9.625 diameter casing strings is 500 pounds per square inch (psi) The pressure on the 9.625 well head (B) section which is the pressure between the 9.625 and 5.5 diameter casing strings is 1,200psi. The 5.5 diameter casing is shut in. The well head assembly is equipped with a tubing head assembly with an emergency shut down device installed in the sales line downstream from the manual master valves. The sales line and well head is shut in and the wells are not producing on the pad at this time. Downstream from the well head assembly is the production equipment consisting of a gas processing unit along with a production fluid tank farm consisting of (4) steel construction 400 barrel capacity tanks situated inside a polymer lined steel containment dike area. The well pad is protected by an earthen berm and there are fire extinguishers situated at all critical points on the well pad.
1540310656 2018-10-23 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad and lease road is stone construction and in good repair. Signage is posted at the entry point of the lease road and on each well head. Hazard signage is posted at the production facility. There is a gate across the lease road. There is a EQT midstream facility along the lease road at the right front corner of the well pad. The well has been drilled and completed and has 2.875 L-80 tubing installed in it to a depth of 10,107. Express Energy flow back service is on location rigging up and the well is slated to be put into the initial stages of production on 10/31/2018. There is a total of (6) 500 barrel flow back tanks being set for the flow back operation.
