0710657545 2017-07-31 Drill / Deepen / Reopen No Violations On the day of my inspection Pioneer 71 drilled an 8.5 inch diameter hole to total depth of 19,026 feet. Wyoming casing was onsite running 5.5 inch diameter casing. Cementers are expected on location in the near future to cement the 5.5 inch diameter casing. Pad drains are shut and containment appears to be functioning as designed and intended. Met with Compnay man David
1499973952 2017-07-13 Casing String Witnessed No Violations On site inspection at 11:45 am to witness the surface casing cement job Pioneer Drilling rig # 71 on the hole and Wyoming Casing and 0-Tex Cement Service on location. The rig has drilled a 24 diameter well bore to a measured depth of 581 with a mine void rubble zone encountered between the depths of 494 and 513 determined by loss of weight on bit and loss of circulation to surface, all measurement were off of a drilling rig Kelly Bushing heigth of 28.6 above ground level. The rig drilled with a 24 diameter hammer bit to the top of the mine then pulled the drill string and switched over to a 24 tri cone mill tooth bit below the mine for the remainder of the 68 feet drilled below the mine floor. Upon completion of the drilling of the borehole the rig pulled the drill string and ran the 18.625 J-55 Grade Buttress Thread 87.5 pound per foot casing string with a manually deployed hydraulically inflated spell packer set in the string between the depths of 358 and 378, a float collar was set in the string at 500.17. O-Tex Cement Service cemented the casing by 1st cementing the bottom shoe track below the mine void by pumping a 45 barrel class H cement mix consisting of 208 sacks of cement weighing 15.6 pound per gallon with the cement weight verified by weight of scale. The cement plug was dropped and displaced with 152 barrels of fresh water. Upon the landing of the cement plug at 300psi the casing went on vacuum and they could not get a pressure test. At that point 287 barrels of fresh water was pumped down the inside of the casing at the casing was determined to be open internally and the plug had failed. So the company loaded a second plug into the cement head and re pumped the job with a additional 80 barrels of cement pumped.
1542204591 2018-11-14 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. All erosion control filter sock material placed on the outward slopes during construction have been removed. The well pad and lease road are stone construction and in good repair. Signage is posted at the lease road entry point and at each well head with hazard warning labels on the tank battery. Emergency shut down devices and fire extinguishers are situated at each critical area of the well pad. There is a 3rd party mid stream operation along the lease road on the left side entering the well pad. The lease road does have a gate to restrict access. A total of 6 wells are present on the pad with the said well equipped with an A&B section well head along with a tubing head production tree. 2 diameter riser pipes with valves and gauges are present and attached to the well head A&B sections for the purpose of annular pressure monitoring. The A section gauge which is monitoring the 13.375 casing pressure reads 600 pounds per square inch pressure (psi) the B section gauge which monitors the 9.625 diameter casing pressure reads 1,200 pounds pressure. The 5.5 diameter casing pressure is 2,100 pounds pressure and the tubing head pressure before the choke valve is 1,600 pounds pressure. The production tubing well head valves are open to the sales line and an emergency shut down device is situated on the production well head tree. There are (6) gas processing units present on location and on the rear left hand corner of the well pad is the production fluid facility that consists of (4) steel construction 400 barrel fluid volume tanks along with a 50 barrel steel tank. All situated inside a polymer lined steel containment dike area.
