0765133246 2018-03-13 UrbanProduction Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 50 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 1300 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . There was a CS Trucking tank on site pulling fluid from the production tanks. The production records within RBDMS are currently updated to 2017.
0815943290 2017-08-22 Completion Testing No Violations The company is laying down matting and preparing this site for completion efforts.
0929608023 2018-03-13 Production Wells No Violations
1495363661 2017-05-18 Casing String Witnessed No Violations On site to witness running and cementing of 18.625 surface casing Nomac rig 137 drilled to a depth of 378 of 24 hole and ran 378 of casing and Stingray cemented it in place with600 sacks of cement and they had good returns and cement stayed up good.
1502042863 2017-06-16 Preliminary Restoration No Violations On the day of my inspection the site passed preliminary restoration per ORC 1509.072A.
1598354200 2018-07-30 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a three-string casing design with 5.5-inch diameter production casing and 2.875-inch diameter tubing that was open flowing. Annular pressure monitoring was in place for the 9.625-inch diameter intermediate casing gauge read 1400 pounds per square inch, and the 13.375-inch diameter casing gauge read 90 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and 4- 400 barrel production fluid tanks, and a 50 barrel produced water tank shared with 34013211010000, 34013212290000, 34013212300000. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.

1742148885 2018-08-23 UrbanProduction Wells No Violations Status check inspection found the well pad perimeter boundary protected by an 8 chain link fence that is gated and locked. There is also a gate on the lease road with signage posted at the well pad entrance points and at each well head with hazard warning labels on the tank battery. The lease road and well pad surface are stoned and in good repair. The well head assembly consists of a lower A section and upper B section with 2 diameter riser pipes on each section access ports with valves and gauges present to monitor annular pressure. The A section gauge which monitors the pressure between the 13.375 and 9.625 diameter casing strings reads 20 pounds pressure and the B section gauge which monitors the pressure between the 5.5 and 9.625 casing strings reads 1,400 pounds pressure. The 5.5 diameter casing internal pressure is 1,800 pounds and the production flow line downstream of the well head production tree and choke reads 1,100 pounds pressure. The wall head production tree has multiple manual shut down and one emergency shut down valve with emergency shut down control points at all critical areas on the well pad including the entry point. There are a total of 4 wells producing on the pad and 4 gas processing units. The production fluid storage area consists of (4) steel construction 400 barrel fluid capacity tanks and one single smaller tank approximately 70 fluid barrels volume, all situated inside a polymer lined steel containment dike area. Also present in the production dike is a charcoal filter. At the entry point of the pad is a midstream facility operated by EQT.
