0065723621 2018-12-26 Completion Testing No Violations Status check inspection on the well completion process found the well drilling and stimulation process complete. The 5.5 diameter production casing was set to a depth of 17,462 . The well was stimulated in 39 stages and the 40 completion plugs have not been drilled out. Prior to drilling out the completion plugs the (2) master valves used in the completion process were changed out and (2) company owned master valves were installed on the well head B section. No production tubing will be set in the well bore and a production tree was installed on the master valves prior to drilling out the completion plugs. The company will not flow the wells back until the completion drill out is finished on all (7) wells on the pad. Deep Well Services tubing snubbing unit is the rig on location completing the wells. All completion equipment is situated on polymer containment. The production equipment has not been set on the pad.

1257399898 2018-11-05 Completion Testing No Violations Status check inspection on the well completion process found that there are 7 wells present on the pad all with the drilling process complete. Halliburton is on site and has stimulated stage 12 of 39 total stages on the said well, also the # 14 well on the pad is currently being completed. EP Wireline service is on location and is in the process of perforating stage 13 of the said well. The stage will be perforated between the depths of 14,777 and 14,941 with 6 clusters of 6 shots per cluster. The average treating pressure is 10,600 pounds pressure with a average fluid volume of 9,500 barrels fluid per stage. The company is receiving a large volume of processed fluid per day by the means of water truck, this fluid is being stored on site in an above ground storage tank that has an open top. The fluid is production water from other wells in the area owned by the operator. The well head assembly is made up of a Cactus Company lower A and upper B permanent well head sections. Atop the Cactus well head B section is a completion stack rented from a company identified as Silver Creek. The stack is made up of the following starting from the bottom working upward, A manual maser valve followed by a hydraulic master valve then a flow cross. Atop the flow cross is another manual master valve used as the working valve followed by a spacing spool then toped off with the working head assembly (goat head) , all components being 15,000 rated working pressure and 7.0625 diameter. All casing annulis are being monitored electronically and all Halliburton completion iron is 15,000 psi rated working pressure.
