0567426033 2017-08-09 Drill / Deepen / Reopen No Violations The rig reported a stain of the soil approx 2 in diameter indicating that the primary containment might be leaking. They took up the mats and found several small holes in the containmnet. They patched the holes and put down another layer of containment and made a lined ditch to make sure there were no further problems.
0983967506 2017-06-26 Casing String Witnessed No Violations On site inspection to witness the Conductor casing cement job at 13:00 hours along with Express Energy Conductor Drilling rig and Express Energy Cement Service. The job was delayed until 17:00 hours due to logistical issues with the water trucks not being scheduled. Express Energys conductor rig has drilled a 36 diameter borehole to a measured depth of 100 from ground level with fresh water encountered at 80. The rig then inserted 100 of .375 wall casing into the borehole. The casing was cemented with 57 barrels of cement consisting of 275 sacks of class A cement with 2% calcium and .50 pound per sack loss circulation material. Cement calculations were for annular capacity with 50% excess and no shoe track. The cement was displaced to 90 with 75 barrels of fresh water. Cement returns were witnessed to surface with 1 of cement left inside the cellar floor to create a liquid tight seal.
1003926077 2017-07-20 Casing String Witnessed No Violations On location to witnes the cementing of the 20 surface casing. The rig ran 20 casing with all the required float equipment to 502 (shoe depth). O-Tex Pumping cemented with 785 sacks (169 bbls) of Class H cement. 30 Bbls of good cement was returned to the surface. The casing pressure test passed. See the casing ticket for the details.
1685124111 2018-06-21 Completion Testing No Violations Status check inspection found a service provider identified as Keane Energy on location with their pressure pumping units (17 total) along with 2 pump down trucks and a wireline truck. The said well has a total of 50 stages stimulated. The well head completion stack is owned by Cactus and consists of a lower master valve followed by a hydrologic master valve then a flow cross with an additional manual master valve atop the flow cross section. Both the 13.375 and 9.625 well head sections have pressure monitoring devices installed.
1732289469 2018-07-11 Completion Testing No Violations On site inspection accompanied by ODNR Inspector Ryan Armstrong found the well drilling and stimulation complete. A coil tubing completion drill out unit owned by Key Energy Services is rigged up on the hole. The Key Drill out pump unit is circulating with 9.3 pound per gallon brine. The 5.5 production casing was set at 19,861. The lower section well head assembly is a General Electric (GE) with the completion stack being a Cactus Well Head consisting of a 7.0625 lower mechanical master valve followed by a 7.0625 hydraulic emergency shut down master valve. There is a flow cross above the 2nd master valve consisting of 2 mechanical master valves and a emergency shut down valve. Multiple completions are taking place simultaneously on the pad with a equipment being placed on polymer containment. There are 5 active wells on the pad. The company man on site Clifton Burton indicated the 9.625 casing well head section was open to the atmosphere however the 13.375 diameter casing well head section was not being monitored. I provided the company men on site a copy of the well construction rules regarding annular pressure.
