0220797706 2017-06-28 Drill / Deepen / Reopen No Violations This is the pad/well that Patterson 296 is going to move on after completing the laterals on the Rammunition pad. Wanted to find the pad prior to the move.
0374911208 2018-06-21 Completion Testing No Violations Status check inspection found a service provider identified as Keane Energy on location with their pressure pumping units (17 total) along with 2 pump down trucks and a wireline truck. The said well has a total of 47 stages stimulated and the process is complete. 4 other wells are being stimulated on the well pad.
0409515991 2018-07-11 Completion Testing No Violations Status check inspection accompanied by ODNR Inspector Ryan Armstrong found the well drilling and stimulation process complete. Consolidated Well Service workover rig was currently on location and performing a completion drill out. EQT company man Clifton Burton informed me that they were monitoring the 9.625 inch diameter deep intermediate casing on the wells and currently were open to atmospheric conditions, however the 13.375 inch diameter surface casing was not being monitored. The well was equipped with a 4 string design with 20 inch diameter surface casing, 13.375 inch diameter intermediate casing, 9.625 inch diameter deep intermediate casing and 5.5 inch diameter production casing. Completion equipment included a 7.0625 inch diameter 17,000 pound per square inch Cactus manual master valve, and 7.0625 inch diameter 17,500 pound per square inch Cactus secondary hydraulic master valve, and a flowback T. Each side of the flowback T had 2- 3 inch diameter manual valves and an emergency shut in device. Lightning Fluid services were on location with their pumping services assisting the drill out process using 9.3 pound per gallon brine.
0913479718 2019-03-28 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.72 with the area affected by the well site construction restored, vegetation is established and erosion control is in place. The well pad surface and approach is stone construction and in good repair. 5 wells are present on the pad and in production, all wells are selling off the 5.5 production casing through (2) primary master valves with emergency shut downs in the production outlet flow line. The wells are selling through sand trap separators with a temporary choke system attached to the sand trap separator. The sand trap separators are being evacuated into a single 500 barrel steel horizontal tank set in polymer containment. Permanent production equipment includes (5) gas processing units along with a production facility consisting of (6) steel 400 barrel tanks set inside a polymer lined steel containment dike area. Also present is a 3rd party mid stream facility.

1507904141 2017-10-13 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson rig 296 walking from this well to permit 3401321195. They anticipate operations to begin on that well by 10/15/17 due to rig maintenance. The containment near the backyard mixing boxes has a tear. The Rice Energy company man indicated that they have a contractor scheduled to come out today to work on their containment. All of the chemicals were within containment. The pad is surrounded by an earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
