1491826363 2017-04-10 Casing String Witnessed No Violations The purpose of my inspection today is to witness the cementing of the mine string. Schlumberger was on location to cement 340 feet of the 18.625 inch diameter casing within a 24 inch diameter borehole. A pre-job safety meeting was held with all personnel on location. Schlumberger pumped 20 barrels of gel spacer, 10 barrels water, and 157 barrels of cement at 15.6 pound per gallon. Cement sample was pulled and weighed in pressurized scales. Top plug was dropped and displaced with 91 barrels of water. 25 barrels into displacement the gel spacer circulated to surface and circulation was maintained for the remainder of the job bringing roughly 51 barrels of cement to surface. Returns were witnessed in the cellar and a sample of the returns was taken.

1809853261 2018-06-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625 inch diameter intermediate casing gauge read 1085 pounds per square inch, the 13.375 inch diameter surface casing gauge read 40 pounds per square inch. Production equipment included a Line Heater, Sand Separator, and Gas Processing unit. There are 2- 500 barrel stock tanks shared with 34013211030000, 34013210930000, 34013210940000, 34013211280000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
