0368256547 2019-02-06 Preliminary Restoration No Violations Preliminary restoration status check inspection found well site restoration meets ORC 1509.072 with vegetation established and erosion control in place. The lease road and well pad surface are stone construction and in good repair. 3 wells are present on the pad. Signage is posted a the lease road entrance and at each well head. Emergency shut down devices are present at each critical area of the well pad.
0570518841 2018-08-29 Production Wells No Violations Status check inspection found 3 wells present on the pad all in production. The lease road has proper identification posted at the entry point and has a gate across it at the well pad entry point. The lease road and well pad are of stone surface construction and in good repair. The well pad perimeter is protected by an earthen berm. There is a Mark West mid stream pipeline station along the lease road at the right front corner of the well pad. There is 3 gas processing units on the left side of the pad and the tank battery is at the left rear corner. The tank battery consists of (4) steel construction 400 barrel capacity tanks and one 50 barrel steel tank, all situated inside a polymer lined steel containment dike area. Hazard warning labels are posted on the tank and there is emergency shut down devices situated at all critical areas on the well pad including at the entry point gate. The well head assembly consists of a lower A section and upper B section with 2 diameter riser pipes attached to both section outlet ports with valves and gauges present to monitor annular pressure. The A section gauge which monitors the pressure between the 9.625 and 13.375 diameter casing string reads 150 pounds pressure. The B section gauge which reads the pressure between the 5.5 and 9.625 diameter casing strings reads zero pounds pressure. The 5.5 diameter casing internal pressure gauge reads 1,750 pounds pressure. There is tubing in the well and the tubing head gauge reads 500 pounds pressure and the production flow line downstream of the choke reads 400 pounds pressure, the well is open flowing up the tubing. Identification is present on each well head.
0654318559 2018-06-19 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing. Annular pressure monitoring was in place, the intermediate casing gauge read 0 pounds per square inch, the surface casing gauge read 150 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit, 4- 400 barrel purge tanks, and a 50 barrel produced water tank shared with 34013211850000, 34013211860000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.

1493434579 2017-04-28 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job with H&P rig 608 on location along with Stingray Cement Service and Wyoming Casing Service. H&P Drilling has drilled a 17.5 diameter well bore to a measured depth of 1,531. Upon reaching total depth a loss circulation 50 barrel gel pill with 10 pond per barrel cedar fiber was pumped and the well bore was loaded with freshwater with the well bore circulating. The drill string was pulled and the 13.375 54.5 lb/ft J-55 grade buttress thread casing ran with the casing being loaded every 5 joints going in the hole. The fluid level continually dropped back and took 90 barrels of fluid to establish circulation each time the well bore was loaded. Stingray Cement Service cemented the casing by 1st pumping a 25 barrel 6% gel spacer followed by 286 barrel cement slurry mix consisting of 1342 sacks of class A cement with 1% calcium and .25 lb/sk loss circulation material, 30% open hole cement was pumped. The cement was displaced with 230 barrels of recycled fluid. Cement returns were witnessed to surface inside the cellar and the casing was pressure tested to 500 psi over displacement pressure upon the landing of the cement plug.
