0190006667 2018-05-24 Completion Testing No Violations Status check inspection on the current well completion process found a stimulation and wireline company on site identified as FTS. The lower well head assembly is GE (General Electric) the completion stack assembly is a company identified as Silver Creek. The company man on site is Walt Arnold. To date a total of 14 of 65 proposed total stages have been stimulated with the completion plug set on the 15th stage at a measured depth of 20,582 and the stage perforated with 6 clusters of 6 shots per foot between the depths of 20,567 and 20,398. Each stage is being stimulated with an average of 9,300 barrels of fresh water and 600,000 pounds of sand. The company is monitoring the 5.5 x 9.625 annular casing pressure however no gauges were found on the 13.375 well head section. The company men on site were advised to monitor all annular pressures while stimulating and completing operations are being conducted.
0198559268 2017-11-29 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Patterson 296 drilled a 24 inch diameter open hole to a depth of 385 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Costys ran 348 feet of 20 inch diameter casing, j-55, 94 lbs/foot with 5 feet of stick up. Casing was centralized with 2 centralizers. Baskets set at 277 feet. Coal encountered from 297-304 feet. Shoe track is 44 feet long. O-tex was onsite with 635 sacks of class H cement. Additives include 3% CaCl and 0.2% X-air. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 104 barrels of fresh water. 35 barrels of gel pumped ahead of cement with one pound per barrel celloflake. 24 barrels of cement returned to surface, and floats held with 1.5 barrels back.
0209412442 2018-06-21 Completion Testing No Violations The said well drilling and stimulation process is complete, the well was stimulated in 65 stages. There is 3 wells on the pad that were recently drilled with 2 stimulated and the 3rd and final well now being stimulated. The company is monitoring all annular pressures.
0494649614 2017-12-04 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson Drilling rig 296 currently drilling their second intermediate at approximately 5247 feet. EQT anticipates reaching their total depth for this string by 12/06/2017. I found multiple areas around the rig and chemical storage containment that had tears and punctures. The EQT company man indicated that he would have a contractor come out on 12/5/2017 to work on their containment. All chemicals are within containment. The pad is surrounded by an earthen dike covered by healthy vegetation. The pad drain valves appear to be closed.
0704732991 2018-08-08 Completion Testing No Violations On site inspection accompanied by ODNR Inspector Ryan Armstrong. Currently there are 4 producing wells on the pad and 3 new wells that the company has service crews on location and completing. The said well is equipped with a lower A section and upper B section with a completion stack present owned by Cameron well company, the well will initially produce through this stack. The production equipment includes 4 gas processing units and 2 glycol heaters. Upon entering the location on the left side of the well pad is the production fluid facility consisting of 6 steel construction 400 barrel fluid capacity production tanks situated inside a polymer lined steel containment dike area. Currently a service rig company identified as Mountain State pressure service is rigged up on the well drilling out the completion plugs at a measured depth of 14,350 with a well bore total depth of 23,199. A total of 64 plugs were set in the well. The well bore is static with 9.3 pound per gallon brine in the hole. No tubing will be set in the well only a tubing hanger. The company is monitoring annular pressure.
0910577500 2018-08-08 Completion Testing No Violations On the day of my inspection I witnessed that Mountain State Pressure Systems was currently on location with a snubbing rig. After speaking with company man Matt Bradford, he informed me that they were currently drilling the 19th plug at 14,350 feet of 69 total isolation plugs to be drilled out. They currently had 9.3 pound per gallon fluid in the hole and well appeared to be static. The total depth of the well was 23,200 feet. I informed Matt that they needed to continue to monitor their annular pressure throughout the completion process. There were four existing producing wells on location that had steel covers over them.
0963048063 2018-07-24 Completion Testing No Violations Status check inspection accompanied by ODNR Inspector Ryan Armstrong found the drilling and stimulation process now complete. There are 2 primary master completion valves present on the well head assembly one manual and one hydraulic, both valves are owned by a company identified as Silver Creek. Annular pressure monitoring is in place with zero pressure between the 5.5 diameter casing and the 9.625 diameter casing and zero pressure between the 9.625 diameter and 13.375 diameter. The well will have the 64 completion plugs drilled out in the near future with a service rig owned by Mount State Pressure Services on location and rigging up to the # 5-H well on location. All equipment is placed on polymer containment.
1692148907 2018-01-12 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 296 drilling an 8.5 inch diameter hole at a depth of 23294 feet. Target total depth for this well is 23400 feet.
1875233884 2017-10-26 Casing String Witnessed No Violations On the day of my inspection I witnessed Express had augured a 36 inch diameter hole to 100 feet. 100 feet of 30 inch diameter conductor casing was run. Express was onsite with 275 sacks of class A cement. Additives include 1.5% CaCl and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 30% percent excess ran., 10 feet left in the casing. Cement was displaced with 8X barrels of fresh water. 15 barrels of cement returned to surface. Water trucks were not on time.

1979075411 2019-01-24 Production Wells No Violations On site inspection found the said well now in production and selling off the 5.5 diameter production casing through (2) primary master valves. The casing pressure is being monitored off a electronic transducer. No annular pressure monitoring is in place. 2 Crew Men from EQT are on site replacing the emergency shut down master valve hydraulic fluid. 3 wells were recently drilled and completed and 4 pre existing wells. Production equipment consists of 5 gas processing units along with a (6) steel 400 barrel capacity steel tanks situated inside a polymer lined steel containment dike area. The well pad perimeter is protected by an earthen berm however one section was removed near the mid stream equipment and not put back. At the rear of the pad is the mid stream operation operated by EQT. I contacted ODNR Engineer Technician Anthony Carson and he will address the issue with the perimeter berm. We have addressed this issue one before and it was addressed. The Preliminary Restoration is due however recent disturbance around the well pad caused by mid stream installation shall delay the restoration until proper vegetation is established. The well pad and lease road surface is stone and in good repair.

2108414721 2018-07-24 Completion Testing No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that there was a completion stack on the well head with a manual master valve and hydraulic master valve present. Annular pressure monitoring was in place and the 13.375-inch diameter casing had 0 pounds per square inch and the 9.625-inch diameter casing had 0 pounds per square inch, the 18.625-inch diameter casing was also being monitored and was at 0 pounds per square inch.
