0418087119 2017-11-16 Plug / Plug Back No Violations On the day of my inspection Express energy is onsite to cement this hole back to surface. Express augered a 32 inch diameter hole to a depth of 92 feet and then went back in with a 36 inch diameter auger bit to ream the hole. At 80 feet the bit snapped off. Express attempted to retrieve the bit for 2 days and Rice decided to plug and abandon the hole. Rice is planning on applying for an emergency permit to replace this well. Well was spudded on 11-12-17 and the bit broke on 11-13-17. No casing is set .Cellar ring is cemented in and I informed Jim Chrisman, contractor for Rice that it would need removed. Express ran 80 feet of 1 inch diameter tubing to depth of approx 75 feet. They brought 720 sacks of cement (50% excess) and only pumped 600 sacks. Cement to surface. Casing plate was placed on top of cement. Cement used was class A cement with a 1.18 yield.
1533755908 2018-08-08 Final Restoration No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that the well and cellar were no longer visible. According to an inspection by Dylan Pendleton on 11/16/2017 during the drilling process of the conductor a 36-inch diameter auger was lost in the hole and was not retrieved. There was no conductor pipe installed in the well the well bore was cemented to surface and cellar had been removed. According to OAC 1509.072 the final restoration was passed for this well and production equipment will remain on site until the remaining wells on the pad have been plugged and abandoned.
