The well appeared to have gauges monitoring annular pressure but were underneath water and I was unable to read them. Production equipment included an individual Gas Processing unit and 4- 400-barrel production fluid tanks, and a 100-barrel production fluid tank shared with 34013209590000, 34013209580000, 34013211730000, 34013213130000. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag were not present, I tried to contact EQT production superintendent Jared Lowery to inform him that they needed to be monitoring annular pressures and wells needed to be identified. |

There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current. |

The said well has no annular pressure monitoring or identification posted and is being flowed through a vertical sand trap separator with the fluid being ran into a temporary storage area consisting of (2) 500 barrel horizontal tanks. There is 3 gas processing units present on the pad. The permanent production fluid storage area consists of (4) steel tanks capable of holding 400 barrels fluid volume each and one horizontal 100 barrel tank, all situated inside a polymer lined steel containment dike area. |

There are 2 wells on the pad that were previously drilled and completed identified as the Iron Warrior # 2 and # 4 wells. 3 new wells were drilled and are in the initial flow back production stages being produced through choke manifolds and sand trap separators, these wells are identified as the Iron Warrior # 1-3 & 5 wells. |

BJ Cementing was on location to complete the job. I witnessed Patterson drilling rig 582 run the last 4 joints of casing and circulate fresh water to surface while BJ was rigging up on location. The job was proposed to be performed as follows, 35 barrels of 6 percent gel, 10 barrels of fresh water, 405 barrels of class H cement, and 344 barrels of fresh water for displacement. Prior to pumping the job, BJ experience mechanical problems with the pump truck. BJ stated that they were sending out a mechanic truck to make the necessary repairs to the pump truck. Partial job witnessed. |
