0103332674 2018-08-07 Preliminary Restoration No Violations Preliminary Restoration inspection found the well pad surface and lease road has a gravel surface and is in good repair. The area affected by the well pad construction has erosion control in place and vegetation established. There are 5 wells present on the pad all in production. The said well is equipped with a lower A section and upper B section with 2 diameter riser pipes present at the well head and equipped with valves and manual pressure gauges to monitor annular pressure. The 13.375 x 9.625 diameter casing pressure was 280 pounds per square inch (psi) the 9.625 x 5.5 diameter casing pressure was 400 psi. The 5.5 diameter production casing internal working pressure was 1,900 psi and the production flow line pressure was 1,000 psi. There are 5 gas processing units on location. At the right rear corner of the well pad is the production fluid facility consisting of 4 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment dike area. There is also a Mark West midstream station at the right front corner entry point of the well pad. Fire extinguishers and emergency shut down control points are present at all the critical areas of the well pad.

1490234451 2017-03-22 Casing String Witnessed No Violations Status check inspection to witness the surface casing cement job with Pioneer Drilling rig # 71 on location and O-Tex cement service. The rig has drilled a 17.5 diameter surface borehole to a measured depth of 1,760 and ran 1,760 of 13.375 J-55 Grade buttress thread 54.5 pound per foot casing with a float shoe and collar along with 11 centralizers casing equipment ran. O-Tex cement service cemented the casing with circulation established by the drilling rig and the cement company pumping 50 barrels of 6% gel spacer ran followed by 325 barrels of class H cement consisting of 1,520 sacks with a 15.6 pound per gallon weight and 1.20 yield. 30% open hole cement excess was ran. The cement was displaced with 264 barrels of fresh water and the casing was pressure tested to 500 psi over cement displacement pressure upon the landing of the cement plug. Cement returns was witnessed to surface.
