0697006449 2018-08-27 Production Wells No Violations On the day of my inspection I witnessed that the well was currently in flowback operations with 2 flowback sand traps, and a choke manifold, the sand traps were tied directly to the production fluid tanks. The well appeared to have gauges monitoring annular pressure but at the time of my inspection I was unable to read them. Production equipment included an additional sand trap, an individual Gas Processing unit and 6- 500-barrel production fluid tanks, and a 100-barrel production fluid tank that was tied directly into the production sand trap all tanks were shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There was also a small excavator on location that had replaced a well pad drain. There were multiple Emergency Shutoff Devices on location, Identification tag were not present, and production was current.
0851827046 2018-10-28 Production Wells No Violations Status check inspection found 5 wells on location all in the production flow back stages. All 5 wells were equipped to produce off the 5.5 production casing through 2 primary master valves with a single additional emergency shut down valve present in the production tree. All 5 wells are producing through sand trap separators with a production fluid temporary storage are set up on location consisting of (4) 500 barrel horizontal mobile tanks. Of the 5 wells present one annular pressure gauge is present and in order to indicate annular pressure monitoring is in place. No surface casing pressure relief devices are present on any of the 5 wells. During the last inspection on 08/27/2018 the DNR Inspector Ryan Armstrong noted to absence of proper well identification and on this day no well identification has been posted, also noted was the condition of the annular pressure gauges. Polymer containment is present around the well heads that the production flow back equipment is situated on, also present on the polymer containment is a diesel engine powered electrical light plant with no earthen ground present. Also present on location is a production facility consisting of (7) steel production tanks capable of holding 500 barrels fluid volume each. Multiple water trucks were present on location and in question to the destination of the produced water it was indicated they were hauling to the Goliath Well pad and staging at the Razing Kane.
1110188554 2019-03-19 Preliminary Restoration No Violations Preliminary Restoration status check inspection accompanied by ODNR Engineer Tech Anthony Carson found that the area affected by the well pad construction stable with vegetation established and erosion control in place. There is a area of the well pad that has fluid being recovered on the outward slope. To date 11 monitoring wells have been installed on the well pad surface and a 3rd party contract company identified as SE Technologies is on site sampling fluid from the monitoring wells. The employee from SE identified as Steve Gerritsen indicated he would be on site the next 2 days sampling. The Preliminary Restoration will be addressed once the area disturbed by the fluid recover is remediated and all fluid recovery has ceased. Addressing the wells. 5 wells are present on the pad and in production. All have annular pressure monitoring in place. The annular pressure is being monitored by the means of electronic transducers installed on the riser pipes attached to the well head A&B sections. The riser pipes are plumbed into a sales line that is fitted with a choke valve.
1443995464 2018-12-13 Production Wells No Violations On site inspection accompanied by Division Hydrology and Engineering Staff along with Supervisors Brent Bear and Mark Atkinson. Also present was representatives with EQT and Rice Drilling. The purpose of the meeting was to discuss the fluid recover efforts on the well pad slope and well pad surface along with a remediation and stabilization plan moving forward. Currently 5 wells are present in production on the well pad. Since my last inspection the company has installed annular pressure monitoring on the well head A&B sections. The pressure is being monitored with electronic gauges attached to 2 diameter riser pipes. The riser pipes are tied into 2 diameter flow lines that are plumed into the production tanks. Also present at the well head is proper identification. The wells are still producing through a 3rd party contractor choke manifold and sand trap separator. The sand trap separators are being evacuated into (2) 500 barrel production tanks separate from the production facility. Additional production equipment on site is 5 gas processing units and a production fluid facility that consists of (7) 500 barrel vertical production tanks along with one horizontal 100 barrel tank, all situated inside a polymer lined steel containment dike area. The identification on the tank battery reflected EQT as the owner however the well head identification read Rice Drilling. The well pad surface is of stone construction.
1495105076 2017-05-11 Casing String Witnessed No Violations On location to witness the cementing of the 20 surface casing. The rig ran 20 casing with all the required float equipment to 509. The #8 Pittsburg coal was 462-473. O-Tex Pumping cemented the casing with 770 sacks (166 bbls) of Class H cement. Good cement was returned to the surface. The pressure test of the casing passed. 2117091513 2018-11-06 Production Wells No Violations On site inspection accompanied by ODNR Inspector Ryan Armstrong along with Supervisors Brent Bear and Mark Atkinson. The purpose of the inspection was to review the water being recovered from the North outward slope of the well pad. It was observed that a 3rd party contractor was on site with portable pumps set up at 4 locations recovering surface fluid from both on and off the well pad, the fluid was being stored in (2) steel construction 500 barrel capacity portable tanks situated on the well pad. I titrate tested the fluid found present in the recovery station points directly on the North side of the well pad at 3 different locations and found no chlorides over 496 parts per million. Addressing the well head assembly, the well is equipped with a lower A and upper B section with no pressure gauges manual or electronic visible on either well head section service outlet ports. There are (2) 7.0625 diameter completion valves install horizontally in line with each other atop the well head B section. Down stream from the master valves is an emergency shut down device. The well is being produced through temporary installed sand trap separators before the product enters the sales line point next to the well head. A gauge present on the well head assembly indicated 4,200 pound pressure on the 5.5 diameter casing. The well was producing off the 5.5 casing.
