1006148723 2018-07-05 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing, which appeared to be open flowing. Annular pressure monitoring was in place the 9.625 inch diameter intermediate casing gauge read 0 pounds per square inch, the 13.375 inch diameter surface casing gauge read 25 pounds per square inch. Production equipment included a Sand Trap, Gas Processing Unit. There were also 4- 400 barrel stock tanks and a 50 barrel produced water tank shared with 34013211300000, 34013211330000, 34013211320000, 34013211290000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.

There are gas processing units on this site. The production area currently contains four 400-barrel production fluid holding tanks. The RBDMS system does not currently indicate production records for this well.

1726404570 2018-08-01 Preliminary Restoration No Violations Status check inspection found the well pad surface and lease road of stone construction, the lease road does have identification posted and a gate present. As you enter the well pad there is a Mark West midstream station on the right side. Situated in the center of the well pad is 5 producing wells all equipped with emergency shut down devices on the production tree outlet side past the 2 primary master valves, the production flow line pressure is 437 pounds per square inch (psi). The 5.5 production casing internal pressure is 1,400 psi. The well head A &B sections is equipped with valves and riser pipes with gauges present to monitor annular pressure. The A section riser pipe which gauges the pressure between the 13.375 and 9.625 diameter casing strings showed zero pounds pressure. The B section which is the pressure between the 9.625 and 5.5 diameter casing strings also had zero pressure showing on the gauge. The wells are selling into gas processing units with a total of 5 units present on location. At the rear left hand corner of the well pad is the production facility consisting of 4 steel construction 400 barrel production tanks situated inside a polymer lined steel containment dike area. Fire extinguishers and emergency shut down control points are situated on the pad at all critical areas. Proper well identification is posted at each well along with hazard identification on each production tank. The well pad earthen berm is in good repair and the perimeter area of the well pad has vegetation established.

2020559708 2018-03-16 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. There were also three 500-barrrel frac tanks on site. Annular pressures read on the gauges provided appeared to be 75 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 0 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . The production records within RBDMS are currently updated to 2017.
