0115672964 2017-12-12 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. There are gas processing units on this site. The production area currently contains four 400-barrel production fluid holding tanks. The RBDMS system does not currently indicate production records for this well.
0657241071 2018-03-16 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. There were also three 500-barrrel frac tanks on site. Annular pressures read on the gauges provided appeared to be 220 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 250 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . The production records within RBDMS are currently updated to 2017.
0999062799 2017-03-01 Casing String Witnessed No Violations On site status check inspection to witness the surface casing cement job found H&P drilling rig # 628 on location. The rig has drilled a 17.5 surface borehole to a measured depth of 1,048 and ran 1,047 of 54.5 lb/ft J-55 grade buttress thread casing with a float shoe and collar casing equipment ran. The casing was cemented by Stingray Cement Service with 215 barrels of Class H cement consisting of 1010 sacks of cement. The cement was displaced to the float collar with 153 barrels of fresh water with 20 barrels of cement returns witnessed to surface and the casing pressure tested to 500 psi over displacement pressure.

1457245332 2018-07-05 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing, which appeared to be open flowing. Annular pressure monitoring was in place the 9.625 inch diameter intermediate casing gauge read 510 pounds per square inch, the 13.375 inch diameter surface casing gauge read 35 pounds per square inch. Production equipment included a Sand Trap, Gas Processing Unit. There were also 4- 400 barrel stock tanks and a 50 barrel produced water tank shared with 34013211290000, 34013211310000, 34013211320000, 34013211330000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
1809462389 2018-08-01 Preliminary Restoration No Violations Status check inspection found the well pad surface and lease road of stone construction, the lease road does have identification posted and a gate present. As you enter the well pad there is a Mark West midstream station on the right side. Situated in the center of the well pad is 5 producing wells all equipped with emergency shut down devices on the production tree outlet side past the 2 primary master valves, the production flow line pressure is 529 pounds per square inch (psi). The 5.5 production casing internal pressure is 2,000 psi. The well head A &B sections is equipped with valves and riser pipes with gauges present to monitor annular pressure. The A section riser pipe which gauges the pressure between the 13.375 and 9.625 diameter casing strings showed 40 pounds pressure. The B section which is the pressure between the 9.625 and 5.5 diameter casing strings also had 500 pounds pressure showing on the gauge. The wells are selling into gas processing units with a total of 5 units present on location. At the rear left hand corner of the well pad is the production facility consisting of 4 steel construction 400 barrel production tanks situated inside a polymer lined steel containment dike area. Fire extinguishers and emergency shut down control points are situated on the pad at all critical areas. Proper well identification is posted at each well along with hazard identification on each production tank. The well pad earthen berm is in good repair and the perimeter area of the well pad has vegetation established.
