0331559436 2017-01-10 Drill / Deepen / Reopen No Violations I received a call from Clay with Rice Energy to notify me that Rice anticipates to spud pad on 01/11/2017.
0415925232 2017-04-27 Casing String Witnessed No Violations On location to witness the cementing of the 20 surface casing. The rig ran the casing with all the required float equipment to 575. O-Tex pumping cemented it with 835 sacks of Class H cement. Good cement was returned to the surface. The pressure test of the casing passed. See the casing ticket for the details.
1474908284 2017-06-28 Drill / Deepen / Reopen No Violations Went to rig and had a walk-around with the company man. No issues were noted. The rig had just walked to this well after setting and cementing the casing on the 6 H well.
1553778855 2019-03-28 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.72 with the area affected by the well site construction restored, vegetation is established and erosion control is in place. The well pad surface and approach is stone construction and in good repair. 5 wells are present on the pad and in production, all wells are selling off the 5.5 production casing through (2) primary master valves with emergency shut downs in the production outlet flow line. The wells are selling through sand trap separators with a temporary choke system attached to the sand trap separator. The sand trap separators are being evacuated into a single 500 barrel steel horizontal tank set in polymer containment. Permanent production equipment includes (5) gas processing units along with a production facility consisting of (6) steel 400 barrel tanks set inside a polymer lined steel containment dike area. Also present is a 3rd party mid stream facility.
1815555828 2018-04-12 Completion Testing No Violations Status check inspection found the well drilling - stimulation and workover process complete. The well is equipped with (2) primary master valves used during the completion process that are in the open position, the valves are attached to the well head (B) section and are in the open position. Atop the master valves is a completion flow cross that is also equipped with a manually operated master valve followed by an remotely controlled emergency shut down valve. Attached to the flow cross is production flow back equipment with manually operated choke manifolds sitting in the area of the well heads controlling the outlet flow and pressure. There is a pressure valve on the flow cross that reads 5,000 (psi) pounds per square inch. On location is a total of 6 active wells all equipped in the same mannerism. Of the 5 wells only one well was equipped with one visual gauge on either of the (2) well heads sections where annular pressure can be monitored, with the (2) well head sections being the (A) section which is the 13.375 casing head section and the (B) section which is the 9.625 casing head section. All (5) cellars were standing full of fluid to surface. Situated on the well pad between the well heads and the production tank facility is a 500 barrel steel portable square tank being used to support the production flow back testing sand traps. It was observed that (2) of the outlet ports were unsecured with no cap of plug in place. The tank was inside a polymer liner that appeared to have recently overflowed with fluid, noted by the fluid level on the tank inside the liner. Attached to the top of the tank is a vent hose constructed of a crushable low pressure poly line, this line is attached at both ends with hose clamps and appears to be connected to a underground production system. Situated at the underground connection point of the tank vent is an electrical sump pump that is 110 volt (AC) alternating current design that is not explosion proof of vapor sealed. The
1876772964 2017-01-11 Drill / Deepen / Reopen No Violations Express Energy has begun the construction of the cellars on this pad. The plan to set all cellars then commence conductor operations following the cellar construction.
