0363166017 2017-09-28 Casing String Witnessed No Violations On location to witness the cementing of the surface casing. The rig ran 304 of 24 79# casing to a shoe depth of 300. O-Tex Pumping cemented it with 465 sacks of Class H cement. 17 bbls of good cement were returned to the surface. The pressure test of the casing passed.
0390936169 2017-09-27 Administrative Inspection No Violations Jon Bechdel of Rice Energy called to report a 10 bbls recycled water spill on Dreamweaver pad. The water never left the containment. The sumps have been shut in as a precaution until the water is cleaned up. It is fresh water that has been down hole and returned. This is the first well on the pad and they just reached total depth on the surface hole so it should not contain any contaminants but those from the fresh water horizons. A clean-up crew is on the way to the rig for remediation. After a visit to the location this date, I found that ~3.5 bbls did get off the containment on the NW side of the pad due to low or broken down curbing . The contaminated material was scooped up by a skid steer and put in a truck and hauled away. The curb/berms were repaired/raised to prevent further issues. The water on the containment was vacuumed up and is still being sucked up as it migrates to the low side of the containment. Cl-
0698822040 2019-01-09 Production Wells No Violations Status check inspection found 4 wells present on the pad and in production. All 4 wells are producing off the 5.5 diameter production casing through 2 primary master valves with an additional emergency shut down valve in line. The wells are selling through 3rd party choke manifolds and sand trap separators. A separate facility is present with 4 steel 500 barrel tanks set inside polymer containment, these tanks are being used for the sand trap separators to be evacuated into. No riser pipes or gauges are present on the well head A&B sections to allow for annular pressure monitoring. Additional production equipment includes 4 gas processing units along with a production facility consisting of (4) steel tanks capable of holding 500 barrels fluid volume each. There is an additional horizontal 100 barrel tank inside the tank battery. All tanks are situated inside a polymer lined steel containment dike area.
0976523394 2018-01-22 Drill / Deepen / Reopen No Violations On the day of ym inspection I witnessed H&P471 drilling their 8.5 inch diameter lateral hole section at a depth of 18,300 feet. They are hoping to be cementing 5.5 inch diameter production casing late thie week. Containment is functioning this date.
1108648621 2017-12-13 Drill / Deepen / Reopen No Violations H & P rig 471 had completed the 12.25-inch diameter open hole portion of the well bore at 9676 feet. It was reported that during displacement of the oil based mud there was a malfunction with a valve that caused a release of oil based mud onto the pad surface. The company was actively removing the oil based mud and materials that had gotten compromised. They were putting them into a roll off box to be properly disposed of.

1441995552 2018-01-18 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed H&P 471 drilling their 8.5 inch diameter lateral hole at a depth of 11,011 feet. Kick off point was at 9,435 feet and curve landed at 10,905. Containment functioning as designed.
1502814789 2017-08-15 Casing String Witnessed No Violations On the day of my inspection I witnessed Express energy had augured a 36 inch diameter hole to 100 feet. 100 feet of 30 inch diameter conductor casing was run. Express energy was onsite with 275 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50 percent excess ran. Cement was displaced with 74.8 barrels of fresh water. 15 barrels of cement returned to surface. No water encountered during drilling.
1800947587 2017-10-13 Drill / Deepen / Reopen No Violations On the date of my inspection I found H&P rig 471 drilling their 12.25 inch intermediate section at approximately 9000 feet. The Rice Energy company man indicated that they would be drilling this section to the Trenton formation at approximately 10900 feet then plug back to the bottom of their 9.625 inch casing string. They are drilling past casing depth to log stratigraphy. All containment appears to be visual free of any tears or punctures. All of the chemicals were within containment. The pad is surrounded by an earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
2084747362 2018-06-20 Completion Testing No Violations Status check inspection found FTS (Frack Tech Services) pressure pumping and perforating services on location and currently simultaneously stimulation the # 5-H and 7-H wells on the pad. 37 of 61 total stages have been completed. All stimulation and support equipment is placed on polymer containment with the well pad perimeter earthen berm serving as the primary containment.
