0404290526 2017-08-15 Casing String Witnessed No Violations On the day of my inspection I witnessed Express energy had augured a 36 inch diameter hole to 100 feet. 100 feet of 30 inch diameter conductor casing was run. Express energy was onsite with 275 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50 percent excess ran. Cement was displaced with 74.8 barrels of fresh water. 15 barrels of cement returned to surface. No water encountered during drilling.
1511943463 2017-11-29 Casing String Witnessed No Violations On the day of my inspection I witnessed the mine string cement job. HP 471drilled a 22 inch diameter open hole to a depth of 650 feet. Previous casing set and cemented was 24 inch diameter surface casing to a depth of 295 feet. Franks ran 617 feet of 18.625 inch diameter casing, j-55, 87.5 lbs/foot with a 5 foot stick up. Shoe track is 42 feet long. Cement Baskets were set at 520 feet. Void encountered bettween 540-550 feet. O-tex was onsite with 665 sacks of class H cement. Additives include 2.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 176 barrels of fresh water. Casing capacity of 165 barrels freshwater pumped and then 35 barrels of gel pumped ahead of cement. No cement returned to surface. Gravel dumped down backside to activate basket. 1 inch diameter tubing ran down the backside to 397 feet. Rig was unable to get deeper and tried for several hours. 7 barrels of cement pumped down backside to set up on basket. An additional 86 barrels of cement was pumped down backside after a half hour wait time. Cement returned to surface.
1512401956 2017-12-04 Drill / Deepen / Reopen No Violations On the date of my inspection I found H&P rig 471 currently tripping in the hole to begin drilling their second intermediate string. I found multiple areas around the rig containment that had tears and punctures. The EQT company man indicated that he would have a contractor come out this afternoon to work on their containment. They are changing the layout of containment areas on the pad since EQT acquired Rice Energy. All chemicals are within containment. The pad is surrounded by an earthen dike covered by healthy vegetation. The pad drain valves appear to be closed and do not appear to be leaking.
1883653158 2019-01-09 Production Wells No Violations Status check inspection found 4 wells present on the pad and in production. All 4 wells are producing off the 5.5 diameter production casing through 2 primary master valves with an additional emergency shut down valve in line. The wells are selling through 3rd party choke manifolds and sand trap separators. A separate facility is present with 4 steel 500 barrel tanks set inside polymer containment, these tanks are being used for the sand trap separators to be evacuated into. No riser pipes or gauges are present on the well head A&B sections to allow for annular pressure monitoring. Additional production equipment includes 4 gas processing units along with a production facility consisting of (4) steel tanks capable of holding 500 barrels fluid volume each. There is an additional horizontal 100 barrel tank inside the tank battery. All tanks are situated inside a polymer lined steel containment dike area.
