After releasing the sheer pin, they were able to drill approximately 14 feet. The milling system which consisted of a window mill, 2 melon mills, an angled downhole motor and heavy weight drill pipe, failed after 14 feet of drilling. Patterson Rig 277 tripped out of the hole to change out the Milling Bottom Hole Assembly. Chad also informed me that they intend to drill an additional 10 feet with the Milling Bottom Hole Assembly at which time they will trip out of the hole and change to a Tri-cone bit bottom hole assembly, clean the well bore and drill an additional 40 feet. I informed Chad to keep me updated with the situation and any problems that they encounter. |

Halliburton was on location and had their cement lines and cement head rigged up. I arrived on location and Halliburton had performed a safety meeting and was currently pumping 13.5 pound per gallon tuned mud spacer with a yield of 2.507 cubic feet per sack, requiring 14.62 gallons per sack water mix. Halliburton then pumped an 8-barrel 14.5 pound per gallon class A cement with a yield of 1.106 cubic feet per sack and 4.63 gallons of water mixed per sack. Halliburton then dropped the aluminum dart and pumped an additional 2 barrels of cement dropped a wiper ball and started displacement with 5 barrels of fresh water. Halliburton then shut down displacement and Patterson UTI 277 finished displacement with 12.0 pound per gallon oil-based mud plug was bumped at 1000 pounds per square inch and pressured up to 3600 pounds per square inch. Ed White with Eventure took over the operations and began buy bleeding back the pressure on the drill string, the cement lines and cement head were removed, and Patterson UTI 277 top drive was connected to the drill string and slips were pulled. Patterson UTI then pumped 12.0 pound per gallon oil-based mud at 2 barrels a minute, pressure was increased to 4000 pounds per square inch in the drill string and the cone assembly was then pumped off bottom and pulled at 30 feet per minute. This process was repeated until the cone assembly had been pumped out of the expandable liner at 7,200 feet. After speaking with EQT representative Travis Leger he informed me that they will wait for cement to reach 500 pounds per square inch compressive strength which will be approximately 30 hours. After cement has cured Travis informed me that they plan to perform a formation integrity test to 14.5 pound per gallon mud weight equivalent. |

The rig is currently laying the liner back down and the company has not proposed a plan moving forward. |

At the current time of my inspection the drill string was positioned in the well bore at a current depth of 8,307, the total drilling depth of the 8.750 diameter well bore is 8,349. The company man on site indicated repairs were still being made to the shakers and at some point this evening the would begin to circulate the 60 barrel gas kick out of the well while replacing the gas void with oil based drilling fluid. Once the well is back in balance they will circulate the entire capacity of the well bore and drill string 2 times before opening the blow out preventer and pulling the drill string up to the window that has been cut out of the 9.625 diameter casing and circulating another 2 times the hole capacity. Once the deem the well bore static they will pull the drill string and puck up an additional reamer and heavy weight drill collar, and return into the well bore with a total bottom hole assembly of 120 with the intention of additionally reaming the window deviation area of the 9.625 casing string so the 1,100 of 7.625 casing liner may pass through the window to the bottom of the well bore. Once the liner is in place it will be cemented ant the liner set in place. |

I informed EQT consultant Chad May to keep me informed on the situation and any problems that they encounter. |
1832380012 | 2019-05-30 | Drill / Deepen / Reopen | No Violations | The purpose of my inspection was a response to a reported incident by EQT Drilling Superintendent Nick Cerone who informed me that they had experienced a gas kick after circulating on the well bore through a roller reamer bottom hole assembly for approximately 15 minutes and there was an ignition at shakers. On the day of my inspection accompanied by District Supervisor Eric Conrad I witnessed the fire on the shakers had been extinguished and there was minimal damage to the wiring and shaker motors. After speaking with EQT company man Travis Leger he informed me that they had tripped out of the hole with the expandable liner on 5/29/2019 and had tripped in the hole with the roller reamer bottom hole assembly which consisted of three drill collars, two three-point reamers and two additional subs. Travis informed me that they did not witness any flow on the well while they were out of the hole and were loading the hole with 12.2 pound per gallon oil-based mud as they tripped in. When they reached the total depth of the well bore at 8,349 feet, they turned on the mud pumps and took a kick. Travis also informed me that they were able to extinguish the fire with the onsite fire extinguishers and immediately called 911. Belmont County fire department showed up and continued to monitor the temperature of the shakers before releasing the pad back to EQT. Travis informed me that they had contractors in route to repair the damaged equipment. I informed Travis that if there was a reportable incident to contact the Ohio one call line and provided him with the reportable incident reference form information. The pressure on the well was 800 pounds per square inch, the annular pressure on the 13.375 inch diameter casing was 0 pounds per square inch. |

The rig has ran 4.5 drill pipe in the hole to a depth of 7,100, there is a 8.5 diameter tri cone tooth bit on the end of the drill string and Schulumberger is on location and preparing to circulate the oil based drilling mud out of the 9.625 casing so the casing may be bond logged. Historically during April of 2018 the 5.5 production cement job failed and the cement locked up leaving the 5.5 diameter production string full of cement. The drilling rig was Patterson Drilling rig 296 and then proceeded to drill the cement out of the drill string with a 2.875 drill pipe string. The cement was drilled out of the inside of the 5.5 diameter casing to a depth of 8,035. The rig was pulling the workover drill string and was 2,392 off bottom when the rig dropped 5,643 of the workover string. Records indicate part of the workover string was recovered and the 5.5diameter string was cut off and recovered from the well. 2 cast iron bridge plugs and a cement isolation plug were then set inside the 9.625 diameter casing with the plugs being set at 7,435 and 7,410. |
