0252556825 | 2018-01-24 | Casing String Witnessed | No Violations | On the day of my inspection I witnessed Precision Rathole had augured a 42 inch diameter hole to 21 feet and a 36 inch diameter hole to 100 feet. 100 feet of 30 inch diameter conductor casing was run. O-tex was onsite with 515 sacks of class H cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 85% percent excess ran. Cement was displaced with 70 barrels of fresh water. 34 barrels of cement returned to surface. |
0402568503 | 2018-04-02 | Casing String Witnessed | No Violations | On the day of my inspection Patterson Rig 284 had reached total depth in the 17.5-inch diameter open hole section of the well bore at 1361 feet. The 13.375-inch diameter casing was ran to a depth of 1351 feet with bow-spring centralizers utilized to provide proper standoff from the well bore. Schlumberger was on site to provide the cementing procedures. There was a safety meeting held to provide the job specifications and to ensure safety of the staff. Following the meeting, Schlumberger pumped 292 barrels of cement and displaced with 201 barrels of water. Once the plug landed the pressure was increased by 500 pounds and the well was shut in for the casing test. The casing test passed. Cement returns to surface were established during the cementing procedure. |
1126527618 | 2018-01-09 | Production Wells | No Violations | This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains two 500-barrel production fluid holding tanks. The production records within the RBDMS system are current to 2017. |

1717180592 | 2018-09-27 | Production Wells | No Violations | On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. The well was still equipped with the completion stack, with 2 manual master valves. Annular pressure monitoring was in place the 9.625-inch diameter intermediate casing gauge read 1015 pounds per square inch, the 13.375-inch diameter surface casing gauge read 5 pounds per square inch. Production equipment included 2 Sand Traps, a line heater, and Gas Processing Unit. There were also 2- 500-barrel steel production fluid tanks inside secondary containment, that were shared with the rest of the producing wells on the pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was current in RBDMS. |

1831989013 | 2018-08-23 | Production Wells | No Violations | Status check inspection found the well pad perimeter protected by an 8 chain link fence with signage posted at the lease road and well pad entry points. The lease road and well pad surface are of stone surface construction. There are 5 producing wells on this pad and one well that was spudded and not complete. The said well is equipped with a lower A and upper B section with 2 diameter riser pipes equipped with valves and gauges attached to both sections to monitor annular pressures. The A section gauge which reads between the 9.625 and 13.375 diameter casing strings shows 9 pounds on the gauge. The B section which reads between the 5.5 and 9.625 diameter casing strings reads 246 pounds on the gauge. The 5.5 diameter casing internal pressure is 5,099 pounds pressure. The well is selling off the 5.5 production casing through dual flow lines with 1,509 and 1,717 pounds pressure present on each flow line gauge. The well head completion stack does have multiple surface safety and manual shut down valves in place on the production tree, also (2) manually operated 7.0625 master valves are still present on top of the B section. Addressing the production equipment present- there is 3 line heaters and 5 gas processing units on location. Also present is a production fluid storage area consisting of (2) steel construction 500 barrel fluid volume tanks situated inside a polymer lines steel containment dike area. The well pad is protected by an earthen berm and the drain valves are in the closed position. |
