0176320918 2018-08-23 Production Wells No Violations Follow up inspection to a spill that occurred on the well pad on or before 08/14/2018 found the (2) 500 barrel fluid volume tanks and the containment they were place in has been removed off site. The earthen material identified by the company as affected during the incident has been removed from the surface and placed in portable container boxes. The area off pad has not been addressed and the Division has requested a remediation plan. A violation was issued against 3401321077-0000.

1133112175 2018-08-14 Production Wells No Violations Follow up inspection to a spill that was discovered on the well site on 08/14/2018. Upon inspection it was observed that crews were on site recovering fluid from a pond area next to the well pad entrance. The fluid entered the pond from the well site sometime between 08/10/2018 and when I discovered it on 08/14/2018. The fluid appeared to of leaked from (2) 500 barrel capacity fluid storage tanks that were situated on the well pad between the well head and the production tanks. The tanks were on location and used during the initial production flow back operation stages. The initial product identified on the well pad by visual site by the company was recovered by a mini excavator and skid steer loader. The product is being stored on site until it can be tested properly. The Division has appointed hydrologist Scott Schiner as the contact point lead man to work with the company and address the remediation-clean up and testing process.
1227967431 2018-08-13 Production Wells No Violations On site inspection accompanied on this day by ODNR Inspector Ryan Armstrong. The well pad was equipped with 6 wells total all wells are currently in production. 3 wells were recently drilled and completed and 3 wells were drilled and completed 2014. The said well is producing off the 5.5 production casing through 2 primary master valves. Supportive equipment on site includes 3 gas processing units with 2 sand trap separators at each unit. The production fluid area consists of 3 steel construction 400 barrel fluid volume tanks situated inside a polymer lined steel containment dike area. The well recently was in the initial flow back operation stages and 2 steel construction 500 barrel capacity tanks were still situated on location between the well head and the production tanks. The tanks were placed inside a polymer lined area. While conducting my inspection I noticed that a reddish colored liquid that was on the well pad surface around the tanks and appeared to be leaking from under the liner the tanks were situated inside of. Also noted was the containment had a reddish colored fluid inside it. The fluid near the tanks was then field titrate tested for chlorides with both low and high level Quantab Chloride strip tests with the results showing chlorides exceeding the low range strip test above a level of 637 parts per million and also exceeding a high level strip test of 6,208 parts per million chlorides. A red cast was also visible in a depressed ditch line on the well pad surface. That area was followed a measured distance of 153 to a catch basin on the North West corner of the well pad. The catch basin had a 12 diameter drain pipe that traveled a measured distance of 100 and exited on the west side of the well pad into a defined ditch line. Also noted is the drain system supporting the catch basing has no shut off valve and the North side of the well pad has no earthen berm. 1291601560 2018-07-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a three-string casing design with surface, intermediate and 5.5 inch diameter production casing, the well was currently in flow back operations. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 29 pounds per square inch, the 13.375 inch diameter surface casing gauge read 1 pounds per square inch. Production equipment included an individual Gas Processing unit. Shared production equipment included a dehydrator, 2 phase separators, contact tower, and 3- 400-barrel purge tanks shared with 34013207790000, 34013207780000, 34013210770000, 34013210780000, 34013208700000. The well pad and all equipment were contained in an earthen dike with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.

1531749689 2019-03-06 Production Wells No Violations during the last phase of drilling on the pad (3) wells were drilled and completed identified as the #2 -#4 and #6 wells. All 3 wells are producing off the 5.5 diameter production casing through the (2) primary master valves set on the B section well head during the completion process. The company has also installed 2 diameter riser pipes onto the well head A&B sections, the riser pipes are equipped with valves and both electronic and manual gauges for the purpose of annular pressure monitoring. All (3) wells are selling through gas processing units. The production fluid facility consists of (3) steel 400 barrel tanks situated inside a polymer lined steel containment dike area.
