1178565147 2018-07-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design with 20-inch diameter surface casing, 13.375 inch diameter intermediate casing, 9.625 inch diameter deep intermediate casing and 5.5 inch diameter production casing, the well was currently in flow back operations with 2 sand traps and 2 choke manifolds. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 133 pounds per square inch, the 13.375-inch diameter surface casing gauge read 1.5 pounds per square inch. Production equipment included an individual Gas Processing unit. Shared production equipment included a dehydrator, 2 phase separators, contact tower, and 3- 400-barrel purge tanks shared with 34013210750000, 34013210740000, 34013210730000, 34013210720000, 34013207460000, 34013207470000. The well pad and all equipment were contained in an earthen dike with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.
1251865132 2016-09-15 Casing String Witnessed No Violations On site inspection to witness the conductor cement job with a initial call out for 12:00 pm on this day and while in route I was contacted by the company man who indicated the cement jobs for the (2) conductors to be cemented would be pushed back to 14:30. Upon arrival the 1st job was executed with the cement displaces by 16:45 pm; the company man on site then notified me that the water trucks hauling the fresh water to cement the casing on the said well would not arrive for approximately 1 hr. and 45 minutes. At that time verified that the 36 well bore was drilled to 100 and 100 of 30 casing was to be set and cemented with 240 sacks of class A cement with 2% calcium and .125 lb/sk lcm and displaced to 90. Both Deputy Chief Dave Claus and my direct supervisor Brent Bear were advise of the scheduling delay and my decision to depart the location due to the extended time already committed to attempting to witness the cement job.
1479732951 2016-11-20 Casing String Witnessed No Violations On location to witness the cementing of the surface casing. Nomac Rig 82 ran 20 casing with all the required float equipment to 470. O-Tex Pumping cemented it with 720 sacks of class H cement. Good cement was returned to the surface. See the casing ticket for the details.
1531242778 2018-07-10 Production Wells No Violations On site inspection accompanied by ODNR Inspector Ryan Armstrong. The well site entrance point was marked with signage and the lease road was of gravel construction. There is 2 well pads sitting next to one another back the same lease road with the Gold Digger being the rear pad and the Sonuvadigger being the pad closest to State Route 9. There was recently 5 wells newly drilled on the pad and 2 prior producing wells. The said well was equipped with electronic annular pressure monitoring on both the 13.375 and 9.625 well head sections. The well was selling off the 5.5 production casing with 2 manually controlled 7.187 diameter master valves in place on the well head followed by an emergency shut down valve with multiple shut down point on the well pad. All valves were open and the well was flowing through a choke manifold and sand trap separator with 2 choke manifold and sand trap systems present on each of the 5 recently completed wells that are on flow back. Also present on location is one gas processing unit present for each well supported by a production fluid facility consisting of 5 steel tanks of 400 barrel construction design situated inside a polymer lined steel containment dike area.
