0130546706 2017-09-06 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be in the flow-back phase of well production. The well is piped to two sand separation units and then to four 500 barrel production fluid holding tanks.
0426986985 2018-04-18 Production Wells No Violations Accompanied Belmont County Inspector Andrew Corder, Reference his Report for further details
0686114364 2016-08-30 Casing String Witnessed No Violations The purpose of my inspection today is to witness the 30 inch diameter conductor being cemented in a 36 inch diameter bore hole at a depth of 100 feet. 51.29 barrels of cement were pumped at 15.6 pounds per gallon. During my inspection I was accompanied by ODNR inspector Andrew Corder.
1002070139 2016-09-21 Drill / Deepen / Reopen No Violations My inspection today finds the drilling contractor, Patterson Drilling 349, is tripping in the hole to drill out the 13.375 casing shoe and drill the 12.25 well bore. While on location Rice Energy EH&S personnel informed me of an area of elevated chlorides. A test of the water in the area indicated chlorides in excess of 1600 parts per million. The company was directed to plug the drain out flow pipes nearest to the chlorides until the area is cleaned.
1474205343 2016-09-17 Casing String Witnessed No Violations Patterson rig 349 on site and they drilled 24 hole to 315 and ran 315 of 20 surface casing and O-Tex cemented it in place with 510 sacks cement and they had 12 bbl. Of good cement returns.
1543328679 2018-11-27 Production Wells No Violations Status check inspection found (4) wells present on the location that are in production. The said well is equipped with a lower A and upper B section well head assembly with 2 diameter riser pipes attached to the A&B section service outlet ports. The riser pipes are equipped with valves and gauges for annular pressure monitoring but are not marked as to what well head section they are attached. The tin horn cellar has been back filled with gravel. The annular pressures recorded on this day were 1,800 and 182 pounds pressure. The 5.5 diameter casing internal pressure is 1,800 pounds pressure. The production tree flow line pressure before the choke is 1,100 pounds pressure. It appears as though the 5.5 internal and external pressures are equalized. There is well identification posted. There is (3) gas processing units present on location.
1625419621 2016-09-02 Drill / Deepen / Reopen No Violations I went ot the location with Eric Conrad to meet the Company man. They are stuck in the hole with the 9-5/8 casing at 8465. Since they can circulate, its likely that they are differentially stuck. Total depth of the 2nd Intermediate hole is 9262. They will attempt to spot Black Magic across the stuck area and hope to get the casing loose. If they cannot get it loose, they will cement it where it is at 8465.
