0365856864 2016-09-21 Casing String Witnessed No Violations My inspection today finds the drilling contractor, Pioneer Rig 71, is running 5.5 P-110 casing with a solid body centralizer every third joint. The O-Tex Pumping cement plan calls for 2560 sacks of class H cement and will bring the tail cement to approximately 1000 above the 9.625 casing shoe.
0633763889 2018-07-31 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a three-string casing design with 5.5-inch diameter production casing and 2.875-inch diameter tubing that was open flowing. Annular pressure monitoring was in place for the 9.625-inch diameter intermediate casing gauge read 925 pounds per square inch, and the 13.375-inch diameter casing gauge read 410 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and 4- 400 barrel production fluid tanks, and a 50 barrel produced water tank shared with 34013210610000, 34013210600000, 34013210620000. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.
0688221958 2017-06-23 Production Wells No Violations During this inspection, I was accompanied by Brent Bear and Michael Moreland. We spoke to the well tender during our visit and discussed the wells production as well was the annular pressures noted on the backside. The annular pressure on the inside of the 9.625 inch diameter casing string was 2250 pounds per square inch. The production line from the well went to a gas processing unit also on site. The tank battery area included four 400 barrel production fluid storage tanks. The production is then routed to a scrubbing unit, through another fluid separator, and then to the contact tower before being metered and going to sale. Shane Ullom of Rice Energy stated that Rice takes over the gas sales starting at the scrubbing unit. This production from this well is being metered by both companies.
0701564254 2018-08-22 UrbanPreliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad perimeter is protected by a chain link fence with a gate present at the entry point of the well pad and along the lease road. Identification is present at both gates along with the entry point of the lease road. There is a midstream pipeline service point at the well pad entry point on the left side of the lease road. 4 wells are present on the well pad. The well head assembly consists of a lower A and upper B section with riser pipes present on the A&B section that is equipped with valves and gauges to monitor annular pressure. The A section which monitors the pressure between the 13.375 and 9.625 diameter casing strings shows 400 pounds per square inch (psi) on the gauge. The B section which is measuring the pressure between the 9.625 and 5.5 diameter casing strings shows 900 psi on the gauge. The 5.5 diameter casing string internal pressure is 1,200 psi and the production flow line pressure is 1,100 psi. The production tree consists of 3 manually operated valve and one emergency shut down valve. The well open flowing. Production equipment on location consists of 4 gas processing units along with a production fluid facility made up of 4 steel design tanks capable of 400 barrels fluid volume each tank. Emergency shut down control switches are present at all critical areas of the well pad.
0898351862 2016-09-02 Casing String Witnessed No Violations O-Tex is still preforming top off efforts. They are still pumping 5 barrel stages to get a base to top off from. After pumping the 5 barrels of cement they attempted to tag the cement. They have been unable to tag any cement at this time. They will continue to pump cement in stages until cement to surface is established.

The pad area is now enclosed with chain linked fence that is gated and locked. This well is equipped with 5.5-inch diameter production casing. The well is free flowing as it does not appear to have any artificial lift technology present at this time. The well is piped to a gas processing unit and then to a production area. The production area contains four 400-barrel production fluid holding tanks. The production records within RBDMS are current to 2017.
1450358182 2016-09-07 Drill / Deepen / Reopen No Violations During this site visit I was accompanied by Inspector Austin Gutridge. Pioneer 71 was on site drilling in the 12.25 open hole section @ 3429. Once total depth is reached the 9.625 casing will be ran and cemented.
1472734482 2016-09-01 Casing String Witnessed No Violations I am on site to witness the running and cementing of 18.625 inch surface casing. Pioneer Energy Services rig 71 drilled a 25 inch diameter hole to a depth of 355 feet. When running casing they had refusal with 1 joint left. They attempted to wash down the casing and were able to get it down to 334 feet, which is approximately 33 feet below a void they encountered from approximately 286 feet to 301 feet. The 18.625 inch casing weighs 87.5 pound per foot casing. O-
1473889292 2016-09-13 Drill / Deepen / Reopen No Violations The purpose of my inspection today is a status check on drilling operations. During my inspection rig Pioneer 71 was on location drilling at a depth of 9085 feet. I was accompanied by ODNR Inspector Andrew Corder.

Annular pressures read on the gauges provided appeared to be 600 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 1100 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . The production records within RBDMS are currently updated to 2017.
1528553165 2017-12-11 UrbanProduction Wells No Violations I was on site to follow up on the fencing that Gulfport said they would provide around the well-head and tank battery. I contacted representative Blake Arthur to inquire on why the fencing has not been installed. Blake informed me that Gulfport plans to have the fencing installed by Christmas of this year. I will follow up at a future date to confirm compliance.
