0115007047 2018-03-13 Urban Drill / Deepen / Reopen No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 450 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 250 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . The production records within RBDMS are currently updated to 2017.
0162440440 2016-08-10 Casing String Witnessed No Violations On site to witness casing being run and cemented in place, they drilled to a depth of 358 of 24 hole and ran 358 of 18.625 casing and O-Tax cemented it in place with 530 sacks of Class H cement and they had 16 bbl. Of good cement returns to surface and casing was equipped with one guide shoe and one float collar and six centralizers.

0661913981 2017-06-23 Production Wells No Violations During this inspection, I was accompanied by Brent Bear and Michael Moreland. We spoke to the well tender during our visit and discussed the wells production as well was the annular pressures noted on the backside. The annular pressure on the inside of the 9.625 inch diameter casing string was 2250 pounds per square inch. The production line from the well went to a gas processing unit also on site. The tank battery area included four 400 barrel production fluid storage tanks. The production is then routed to a scrubbing unit, through another fluid separator, and then to the contact tower before being metered and going to sale. Shane Ullom of Rice Energy stated that Rice takes over the gas sales starting at the scrubbing unit. This production from this well is being metered by both companies.
0810821360 2018-07-31 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a three-string casing design with 5.5-inch diameter production casing and 2.875-inch diameter tubing that was open flowing. Annular pressure monitoring was in place for the 9.625-inch diameter intermediate casing gauge read 200 pounds per square inch, and the 13.375-inch diameter casing gauge read 100 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and 4- 400 barrel production fluid tanks, and a 50 barrel produced water tank shared with 34013210640000, 34013210600000, 34013210620000. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.

1002092592 2016-07-10 Casing String Witnessed No Violations 07/09/2016- On site inspection to witness the conductor casing cement job found Butches Rat Hole on location with a 90 x 36 conductor borehole being drilled and 90 of 26-.375 wall weld connection conductor pipe being ran. Stingray Cement service cemented the casing using 540 sacks of class A cement with circulation established prior to cementing and cement returns witnessed to surface at 40 barrels into a total of 56 barrels displacement. There was a unconsolidated area hit between the depths of 30 and 50 during drilling and solid limestone bedrock at 88.
1156094354 2016-08-15 Drill / Deepen / Reopen No Violations Pioneer 71 was drilling @ 3799 in the 12.25 open hole section of the well bore. Once total depth of this section is reached the operator will run and cement the 9.625 casing.

The pad area is now enclosed with chain linked fence that is gated and locked. This well is equipped with 5.5-inch diameter production casing. 1472156668 2016-08-25 Drill / Deepen / Reopen No Violations The purpose of my inspection today is a status check on drilling operations. During my inspection the rig was at a depth of 14,180 feet. I was accompanied by ODNR inspector Andrew Corder.
1534957488 2018-08-22 UrbanPreliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad perimeter is protected by a chain link fence with a gate present at the entry point of the well pad and along the lease road. Identification is present at both gates along with the entry point of the lease road. There is a midstream pipeline service point at the well pad entry point on the left side of the lease road. 4 wells are present on the well pad. The well head assembly consists of a lower A and upper B section with riser pipes present on the A&B section that is equipped with valves and gauges to monitor annular pressure. The A section which monitors the pressure between the 13.375 and 9.625 diameter casing strings shows 900 pounds per square inch (psi) on the gauge. The B section which is measuring the pressure between the 9.625 and 5.5 diameter casing strings shows 250 psi on the gauge. The 5.5 diameter casing string internal pressure is 1,500 psi and the production flow line pressure is 1,050 psi. The production tree consists of 3 manually operated valve and one emergency shut down valve. The well open flowing. Production equipment on location consists of 4 gas processing units along with a production fluid facility made up of 4 steel design tanks capable of 400 barrels fluid volume each tank. Emergency shut down control switches are present at all critical areas of the well pad.

I contacted representative Blake Arthur to inquire on why the fencing has not been installed. Blake informed me that Gulfport plans to have the fencing installed by Christmas of this year. I will follow up at a future date to confirm compliance.
