0443747840 2016-09-28 Casing String Witnessed No Violations The purpose of my inspection today is to witness 412 feet of the 20 inch diameter surface casing being cemented into place in a 24 inch diameter borehole. During my inspection I witnessed O-Tex test their lines at 2500 pounds per square inch. Following the pressure test O-Tex pumped 150 barrel of water, 35 barrels of gel, and 2 barrels of fresh water. O-Tex then pumped 134 barrels of Class H cement at 15.6 pounds per gallon. O-Tex pulled a sample of the cement and weighed the sample in separate scales for my verification. Results from weighing the cement came back as 15.6 pounds per gallon. The plug was released and displaced with 126 barrels of water. I witness cement flowing into the half round that was being used for returns. 19 barrels of cement returns were established.
1238833227 2018-09-19 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter casing, Annular pressure monitoring was in place on both the 9.625-inch diameter casing gauge read 1230 pounds per square inch, the 13.375-inch diameter casing gauge read 0 pounds per square inch. Pressure release lines have been installed from the 9.625-inch diameter casing to the production fluid tanks, the 13.375-inch diameter casing had pressure release lines that vented to atmosphere. Production equipment included 2 sand traps for each well an individual Gas Processing unit, and 4- 400-barrel production fluid tanks shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There was flow back equipment still on location including 4 additional sand traps and 8- 500-barrel mobile fluid storage tanks on location with secondary containment, flowback equipment was no longer attached to the well head. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current.

2113038761 2019-03-21 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with erosion control in place and vegetation established. The perimeter sediment control filter sock has been removed. The well pad and lease road surface is stone construction and in good repair. The well pad perimeter is protected by an earthen berm. 4 wells are present and producing on the well pad, the wells are equipped with production trees with emergency shut down devices in place. Annular pressure monitoring is also installed on the well head A&B sections. Proper identification is posted on the well heads with hazard warning labels on the production tanks. There are (4) 400 barrel tanks present inside a polymer lined steel containment dike area. 4 gas processing units are present on the pad. I contacted Bruce Gallick with the company and asked that he address the plugging and cutting off 30 below grade of the Mouse Holes.
