0109975091 2017-10-31 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains five 400-barrel production fluid holding tanks. Production records for this well are current to 2017 within RBDMS.
0493772854 2016-09-27 Completion Testing No Violations I received a call from Rice Energy. Planned operations are to perform the casing test and open the top sleeves on this well.
0517462357 2016-04-06 Drill / Deepen / Reopen No Violations My inspection today finds the drilling contractor, Patterson Drilling 296, is pulling the drill pipe out of the well after reaching a depth of 19,060. I received the 24 hour notice of intent to cement while on location.
0721843171 2016-01-19 Surface Facility Construction No Violations
0781981523 2018-10-18 Production Wells No Violations On the day of my inspection accompanied by ODNR Southeast District Supervisor Mike Moreland I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing with no artificial lift technology. At the time of my inspection annular pressure monitoring was in place and the 9.625-inch diameter casing was at 1075 pounds per square inch, the 13.375-inch diameter casing pressure was 250 pounds per square inch. Production equipment included a Gas Processing Unit, and 3 400-barrel steel production fluid tanks that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
1111058689 2018-03-29 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains five 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 4000 pounds on the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. A gauge for the inside of the 13.375-inch diameter casing and the outside of the 9.625-inch diameter casing is not currently provided. The production records within RBDMS are currently updated to 2017.
1137586689 2016-01-20 Casing String Witnessed No Violations

1972574918 2018-04-09 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains five 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 3693 pounds on the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. There was no pressure rating gauge on the inside of the 13.375-inch diameter casing and outside of the 9.625-inch diameter casing string annulus. The production records within RBDMS are currently updated to 2017.
