0140150738 2016-01-19 Drill / Deepen / Reopen No Violations On my inspection today with MRI Andrew Corder, we found personnel on site placing liner and rig mats in preparation for the arrival of the drilling rig. A spudding unit was on site waiting to begin drilling the initial wellbore for the conductor phase of the casing program. The pad earthen berm was in place. Pad slopes and access road appeared to be in good condition.

1319979485 2018-10-18 Production Wells No Violations On the day of my inspection accompanied by ODNR Southeast District Supervisor Mike Moreland I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing with no artificial lift technology. At the time of my inspection annular pressure monitoring was in place and the 9.625-inch diameter casing was at 320 pounds per square inch, the 13.375-inch diameter casing pressure was 29 pounds per square inch. Production equipment included a Gas Processing Unit, and 3 400-barrel steel production fluid tanks that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
