0816816842 2016-09-21 Drill / Deepen / Reopen No Violations On site inspection to witness the 18.625 surface casing cement job accompanied by ODNR Division staff member Jimmy Wiles. Pioneer Drilling rig # 73 is on location and has drilled a 24 surface hole on air to a depth of 328 off 28 rig Kelly bushing. The rig ran 328 of 18.25 casing with a guide shoe and float collar ran and proper centralizer placement demonstrated. O-Tex cement service is on location to cement the casing using 550 sacks of class H cement with circulation being established to surface prior to cementing and the cement displaced to the float collar with 87.5 barrels of fresh water and the casing pressure tested to 500 psi over cement displacement pressure. Cement returns were witnessed to surface.
1529938696 2018-06-25 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625 inch diameter intermediate casing gauge read 50 pounds per square inch, the 13.375 inch diameter surface casing gauge read 540 pounds per square inch. Production equipment included a Sand Trap, and Gas Processing Unit. There are 4- 400 barrel stock tanks and a 50 barrel produced water tank shared with 34013210070000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.

1683940991 2016-09-21 Casing String Witnessed No Violations On site inspection along with Inspector Bill Stolarik to witness the 18.625 surface casing cement job. Pioneer Rig # 73 is on location and has drilled a 24 surface borehole to 328 and ran 328 of 18.625 J-55 casing. The casing was cemented by O-Tex cement service using 550 sacks of class H 15.8 lb/gl cement with the cement weight verified by weight of scale. Circulation was established prior to cementing with 50 barrels of 6% and the cement displaced to the float collar with 87.5 barrels of fresh water and the casing was pressure tested to 500 psi over cement displacement pressure with cement returns witnessed to surface inside the cellar.
