0474788717 2017-09-20 Production Wells No Violations I responded to a spill reported on this site by Gulfport Energy representative, Brad Burwell. There was a mechanical failure at the gas separation unit and there was an estimated 100 gallons of production fluid dispersed onto the pad surface. The operator had dispatch the cleanup and remediation crews and they were in route. The operator plans to flush the area with fresh water and utilize vacuum trucks to remove the production water. They will screen the area until the water reaches an acceptable level. The outflow drains were shut and will be monitored prior to releasing water accumulated from rain events to ensure that the water is acceptable to be released from the outflow drains.
0969125263 2018-07-31 Preliminary Restoration No Violations Preliminary restoration status check inspection found the area affected by the well pad construction has erosion control in place and vegetation established, the well pad surface and lease road are of stone construction and are in good repair. The entry point of the well pad is gated and locked with proper well site identification posted at the entry point of the lease road. There is an earthen berm established around the well pad and the company is controlling any collected surface water from rain events. There are a total of 5 wells on the pad that are in production at this time. The company has emergency shut down master control buttons and fire extinguishers place at all critical points on the well pad. The well head assembly consists of a lower B section and upper A section along with a production tree assembly. There are riser pipes with valves and manual pressure gauges attached to the A & B section that is allowing the company to monitor annular pressure. The pressure on the A section which monitors the pressure between the 13.375 diameter surface casing and 9.625 diameter intermediate casing string is currently 400 pounds per square inch (psi) and the pressure on the B section which is the pressure between the 5.5 diameter production casing and the 9.625 diameter intermediate casing is 200 pounds pressure. The 5.5 diameter production casing internal working pressure is 1000 psi and the production flowline pressure is 1,100 psi. There is an mechanical lift master control device on the production flowline downstream from the production tree manual control valves and a artificial lift system on the production tree. There is a total of 5 gas processing units on the well pad one for each well. On the back right hand corner of the well pad is the production fluid storage tank farm consisting of (4) steel construction 400 barrel tanks situated inside a polymer lined steel containment dike area.
