0109402830 2015-12-15 Drill / Deepen / Reopen No Violations My inspection today finds the rig lost circulation on the well at a depth of 8471 after drilling thirty feet below the casing shoe. The rig is mixing a 100 barrel 100 pound lost circulation material pill. If they cannot reestablish circulation they will attempt to cement at the base of the 9.625 casing shoe.
0637307109 2015-12-17 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig is preparing to set down on the cement retainer set tool for a second attempt to fish it out of the hole. The retainer tool is at a depth of 8341.
0666642135 2015-12-21 Drill / Deepen / Reopen No Violations My inspection today finds the drilling contractor, Nomac 83, is preparing to install a whipstock in the 9.625 casing at 8154. They will then mill through the casing and resume drilling.
0724073968 2016-01-04 Drill / Deepen / Reopen No Violations On this day the process of rigging down Nomac Rig # 83 was underway. Once the rig down process is complete this rig will transit to the George pad.
0957129881 2018-06-14 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing. Annular pressure monitoring was in place, the intermediate casing gauge read 1000 pounds per square inch, the surface casing gauge read 0 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and 4- 400 barrel purge tanks, and a 50 barrel produced water tank shared with 34013209710000, 34013209720000, 34013209730000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
109402830 2015-12-15 0:00 DD No Violations 3.00
1262618210 2015-12-29 Drill / Deepen / Reopen No Violations My inspection today finds Nomac rig 83 is drilling an 8.5 borehole at a depth of 16,751. Evolution Energy Trucking is on site removing accumulated rain water from the containment.
1268197136 2018-08-01 Production Wells No Violations Status check inspection found the well pad surface and lease road of stone construction, the lease road does have identification posted and a gate present. As you enter the well pad there is a Mark West midstream station on the left side. Situated in the center of the well pad is 4 producing wells all equipped with emergency shut down devices on the production tree outlet side past the 2 primary master valves, the production flow line pressure is 500 pounds per square inch (psi). The 5.5 production casing internal pressure is 500 psi. The well head A &B sections is equipped with valves and riser pipes with gauges present to monitor annular pressure. The A section riser pipe which gauges the pressure between the 13.375 and 9.625 diameter casing strings showed 10 pounds pressure. The B section which is the pressure between the 9.625 and 5.5 diameter casing strings has 650 pressure showing on the gauge. The wells are selling into gas processing units with a total of 4 units present on location. At the front left hand corner of the well pad is the production facility consisting of 4 steel construction 400 barrel production tanks situated inside a polymer lined steel containment dike area. Fire extinguishers and emergency shut down control points are situated on the pad at all critical areas. Proper well identification is posted at each well along with hazard identification on each production tank. The well pad earthen berm is in good repair and the perimeter area of the well pad has vegetation established.
1681997505 2015-12-18 Drill / Deepen / Reopen No Violations I spoke to Nick Sharp, Gulfport Drilling Consultant, about the well. Mr. Sharp stated that a caliper log was performed and indicated that the casing was parted at three joints from bottom. He said that the company would notify the Division once a plan to remediate was developed.
1894460387 2016-10-05 Production Wells No Violations Completion efforts on this pad are finished. Production crews are on this site assembling the production equipment. Once production equipment is finalized the wells are anticipated to begin producing.
2099945626 2015-12-16 Casing String Witnessed No Violations My inspection today finds the drilling rig, Nomac 83, drilled 1150 of 17.5 borehole. The rig then ran 1147 of 13.375 casing. Stingray Cementing cemented the casing by circulating 1030 sks of class H cement around the backside of the casing. I witnessed thirty barrels of cement circulate to surface.
