0376172093 2015-10-09 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. Nomac Rig 83 drilled 393 of 24 hole. The rig then ran 393 of 18.625 J-55 casing with eight centralizers, a float shoe and a float collar. Stingray cemented the casing in place by circulating 126 barrels of class H cement with 2% CaCl and .25 pounds per gallon poly flake into the annular space. I witnessed twenty barrels of cement circulate to surface.
0472296789 2018-06-14 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing. Annular pressure monitoring was in place, the intermediate casing gauge read 0 pounds per square inch, the surface casing gauge read 225 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and 4- 400 barrel purge tanks, and a 50 barrel produced water tank shared with 34013209720000, 34013209730000, 34013209790000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
0628671784 2016-10-05 Production Wells No Violations Completion efforts on this pad are finished. Production crews are on this site assembling the production equipment. Once production equipment is finalized the wells are anticipated to begin producing.
0705538437 2015-11-02 Drill / Deepen / Reopen No Violations My inspection today finds Nomac Rig 83 is drilling a 12.25 borehole at a depth of 7710. Fire extinguishers are staged around the location.
1799596398 2018-08-01 Production Wells No Violations Status check inspection found the well pad surface and lease road of stone construction, the lease road does have identification posted and a gate present. As you enter the well pad there is a Mark West midstream station on the left side. Situated in the center of the well pad is 4 producing wells all equipped with emergency shut down devices on the production tree outlet side past the 2 primary master valves, the production flow line pressure is 500 pounds per square inch (psi). The 5.5 production casing internal pressure is 1,000 psi. The well head A &B sections is equipped with valves and riser pipes with gauges present to monitor annular pressure. The A section riser pipe which gauges the pressure between the 13.375 and 9.625 diameter casing strings showed 180 pounds pressure. The B section which is the pressure between the 9.625 and 5.5 diameter casing strings has zero pressure showing on the gauge. The wells are selling into gas processing units with a total of 4 units present on location. At the front left hand corner of the well pad is the production facility consisting of 4 steel construction 400 barrel production tanks situated inside a polymer lined steel containment dike area. Fire extinguishers and emergency shut down control points are situated on the pad at all critical areas. Proper well identification is posted at each well along with hazard identification on each production tank. The well pad earthen berm is in good repair and the perimeter area of the well pad has vegetation established.
1836867048 2016-02-02 Preliminary Restoration No Violations On this day the production staff was laying pipeline up to the existing well. The site appeared to be in satisfactory condition. Erosion and sediment controls are in place and functioning properly. Preliminary Restoration for this site has passed.
1880377260 2015-11-10 Drill / Deepen / Reopen No Violations My inspection today finds the drilling contractor, Nomac Rig 83, is drilling an 8.5 borehole at a depth of 12,854. ECM trucks are removing rain water from location. I tested water in a puddle on location and at a drainage sump with a Quantab, both tests indicated total chlorides are under fifty parts per million.
