0094937105 2018-08-08 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design with 5.5-inch diameter production casing, and 2.875-inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place on the well with transducers with no ability to read pressures. Production equipment included an individual Gas Processing unit and 4- 400-barrel production fluid tanks, and a 100-barrel production fluid tank shared with 34013209580000, 34013211720000, 34013211730000, 34013213130000. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag was present, and production was up to date. The location also contained a 500-barrel fluid tank, that was hooked to the wells that were currently set up for flowback. The water containment unit was being removed at the time of my inspection.
0455268257 2018-06-21 Production Wells No Violations Status check inspection on the well drilling process found the well is in production and producing off the production tree with 3 primary master valves present and in line. The company has electronic annular pressure monitoring installed on the 13.375 and 9.625 well head sections. The well is selling into a gas processing unit. Also on site is production facility consisting of 4 tanks of steel construction design situated inside a polymer lined steel containment dike area. There are 2 wells on the pad that were previously drilled and completed identified as the Iron Warrior # 2 and # 4 wells. 3 new wells were drilled and are in the initial flow back production stages being produced through choke manifolds and sand trap separators, these wells are identified as the Iron Warrior # 1-3 & 5 wells.
0860311814 2018-11-05 Preliminary Restoration No Violations Preliminary Restoration status check inspection finds the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad and lease road surface is stone construction and in good repair. There are 5 wells total present on the pad with the # 2 & #4 being previously drilled and the #1-#3 &#5 being drilled in the 2nd round. The said well has riser pipes present with electronic transducers for annular pressure monitoring. There is 3 gas processing units present on the pad. The production fluid storage area consists of (4) steel tanks capable of holding 400 barrels fluid volume each and one horizontal 100 barrel tank, all situated inside a polymer lined steel containment dike area. Proper well identification is posted and cages are present over the # 2 and #4 wells.
0958190557 2018-08-08 Production Wells No Violations Status check inspection accompanied by ODNR Inspector Ryan Armstrong found that the well is producing off the 5.5 production casing. There is annular pressure monitoring on the well head A&B sections with electronic gauges present. No manual gauges are present. Cages are around the wells for protection purposes while 3 additional wells were drilled on the pad.

1650395416 2017-09-14 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to three 400 barrel production fluid holding tanks. Elk Lake Services was on site and surveying the well pad. Production records in RBDMS are current to 2017.
